GUIDELINES FOR SURVEYORS ENTERING ENCLOSED SPACES ONBOARD MANNED VESSEL

1. DEFINITION

1.1 An enclosed space onboard a Manned Vessel such as an Anchor Handling Tug Supply
Vessel, Derrick Lay Barge, Dive Support Vessel, Tanker, Bulk Carrier etc., (excluding
unmanned Flat Top Transportation Barge) means any enclosed space not continuously or
adequately ventilated, where the atmosphere may contain toxic or flammable gases or be
deficient in oxygen (Atmospheric Hazards) to the extent of being incapable of supporting
human life.

This is to include but not be limited to Cargo or Ballast tanks, Fuel tanks, Dry Bulk tanks,
Liquid Mud / Brine tanks, Methanol / Base Oil tanks, Duct Keels, Void Spaces, Storerooms,
Boilers, etc.

1.2 Atmospheric Hazards means

- The accumulation of flammable, combustible or explosive agents,


- An oxygen content in the atmosphere that is less than 20.8 percent or more than 21
percent by volume or the accumulation of atmospheric contamination, including gases,
vapours, fumes, dusts or mists that could result inactive health effects that pose an
immediate threat to life or interfere with a person’s ability to escape unaided from
enclosed space.

2. PRIOR TO ENTRY

2.1 Surveyor entering an enclosed space should be trained in enclosed space entry. He should
be aware of the requirements and safety measures on the entry.

2.2 A safety meeting including Job Hazards Analysis (JHA) with all parties concerned should be
held prior to survey to discuss all aspects of safety measures including but not limiting to
the following items:

- Permit to work should be obtained for the space entered and the validity of permit to
be clearly stated.
- Responsibilities and role of each personnel.
- Identified hazards and assess the risks
- All other work that create noise, hot work or other hazard within the space to be
temporarily stopped whilst the survey is ongoing.
- Last content of spaces to be surveyed and the spaces adjacent to it.
- Evaluate that the enclosed space is empty, clean and ventilated.
- Evaluate need for isolation of space.
- Ensure that a standby / and or rescue team is in place.
- Evaluate lighting arrangements.

- The Attending Surveyor(s) must ensure that the space has been adequately ventilated
and verify it is safe to enter by use of an oxygen meter or a certificate issued by a
competent authority.

2.3 Surveyor to ensure that the appropriate oxygen or multi-gas meter he uses to have been
annually calibrated and function tested and confirmed operable prior entry to enclosed
space.

2.4 The Master / Owners / Charterers of the Vessel to be surveyed must be aware of the
Attending Surveyor entry into enclosed spaces.

2.5 No Surveyors from this office shall be permitted to enter an enclosed space if a “stand by”
man is not available at the entry point into the enclosed space.

2.6 An emergency plan is to be prepared, this is to cover what action is to be taken should the
party within the enclosed space collapse or become injured, and include forms of
communication with the local rescue authorities. The emergency oxygen pack should be
available and “standby” man instructed on its use.

3. PREPARATIONS FOR ENTRY

3.1 The Attending Surveyor should ensure ventilations to be continuous with all openings are to
be opened for ventilation including emergency exit.

3.2 The Attending Surveyor should evaluate the need of the isolation of enclosed space before
entering the space. The surveyor should not enter or remain in any ballast or cargo space if
ballast is transferred into or out of that space, if not agreed beforehand.

3.3 The Attending Surveyor shall wear protective clothing in the form of coveralls / boiler suit,
and safety shoes or boots, hand gloves, goggles as appropriate and have a torch light or
other suitable means of illumination.

3.4 All known hazards within the spaces are to be considered and a decision made to determine
if safe entry is possible, if there is any doubt entry shall be postponed until such doubt is
removed.

4. DURING ENTRY

4.1 Communication shall be maintained with the “stand by” man at all times whilst the
enclosed space is being inspected.

4.2 If at any time the Attending Surveyor feels uncomfortable with the atmosphere in the space
or is in doubt as to the quality of the atmosphere he is to exit the space immediately.

4.3 If the Attending Surveyor in the space is injured or collapses the “stand by” man is to raise
the alarm as previously arranged and then take steps to rescue the injured party without
endangering his own life.

5. COMPLETION OF SURVEY

5.1 The Attending Surveyor shall inform the relevant parties of the completion of survey and
reinstate the surveyed unit to the condition it was in upon arrival.

6 OBSERVANCE

6.1 This policy is to be strictly observed and commercial pressures or circumstances shall in no
way interfere with the above.

6.2 If the guidelines cannot be observed and in the evaluated consideration by the Attending
Surveyor that entry into the enclosed space is unsafe, the Attending Surveyor is fully
authorised by the Management to reject the unit until corrective action has been taken.
