Affidavit in Support of Request for the correction of the name

of the husband of the deceased Smt. V Archana Reddy on the

Death Certificate bearing Registration No:-
613063/V/D/2022/000003

I, Yuvika Reddy, d/o Shri R Aravind Reddy and Late. Smt. V

Archana Reddy (Mother’s name) aged about 22years, residing at:-
2255, 15th Cross, 21st Main, Bommanahalli, HSR Layout, Bangalore
South, Begur Village, Bengaluru South, Taluk of Bangalore, State of
Karnataka Bank, do hereby solemnly affirm and declare as follows:-

1. I state that I am the daughter of the late Smt. V Archana

Reddy, w/o Shri R Aravind Reddy who passed away on

28.12.2021.

2. I further state that my mother was declared dead at 12.33 am

on 28.12.2021 at the Kauvery Hospital, Electronic City,
Bengaluru South. Furthermore, I state that in the event of a
death occurring in a hospital, the person who has expired is
entered by the hospital authority in the E-janma portal and
this data is used by the concerned jurisdictional officer for the
generation of the death certificate. That though we had
furnished correct records of Late V Archana Reddy, like Aadhar,
copy of passport etc. which correctly reflect the name of the
husband, an inadvertent error occurred during data entry by
the hospital. As a result, there is an error in the death
certificate.
3. Upon perusal of the said death certificate, I observed that the
name of the husband of the deceased is wrongly mentioned as
“Naveen Kumar V” instead of the correct name “R Aravind
Reddy”.

4. The correction of the name of the husband of the deceased is

essential for dealing with the estate of the deceased.

5. Enclosed herewith are the following documents in support of

the request for the correction of the name of the husband of
the deceased:

1) Copy of the Aadhar Card of the deceased Smt. V

Archana Reddy.
2) Copy of the Attestation of the Family Tree of the
deceased.
3) Copy of the marriage certificate of the deceased.

6. I am also enclosing herewith the death certificate issued by

the concerned authorities, which necessitates the correction of
the name of the husband of the deceased.

7. I hereby affirm that the contents of this affidavit are true and
correct to the best of my knowledge, belief, and information.
