Brief description of Observation cards Company Action Card Category

1 Anti-skid treads leading from the drill floor to the cantilever need to be DD Open .Housekeeping
cleaned , the treads are no longer anti-skid due to dirt and mud, talk held
with supervisor and he agreed to action .

2 Aerosol can left out in direct sunlight, I placed the can in the roustabout DD Raise @ PTSM .Housekeeping
locker , crews to be reminded that aerosols can explode if they over-heat .

3 Work about to start inside tank, it was observed that the visor on BA DD Raise @ PTSM .Housekeeping
mask for the BA rescue set was covered in dust, if the set had to be used
in a emergency, time would been wasted cleaning the visor , safety talk
held and the equipment was cleaned , men agreed to have all rescue tools
and equipment ready for use in future .

4 Hose restricting access to the eye-wash station at the cement unit, I moved DD Closed .Housekeeping
the hose , talk held later with area owner about keeping access to
emergency equipment clear at all times.
5 Some people leaving their dirty gloves on in the change room seat, please PVDL-Slickline Closed .Housekeeping
put the dirty gloves into the glove wash bin and help keep the change
room tidy .
6 On April 3rd landed on the TGT-H1 platform and observed that the only DD Raise with Client .PPE
member of the heli-deck crew that was wearing ear protection was the
HLO, the HLO should ensure the all the men working on the heli-deck are
wearing all the required PPE .

7 Man observed spraying chemicals above his head without wearing safety DD Raise @ PTSM .PPE
glasses, talk held about the risk of the chemicals entering the mans eyes,
as agreement he then continued his task wearing eye protection .

8 Task ongoing without the required JSA having been reviewed and signed , DD Raise @ PTSM .Procedures
safety talk held and men agreed that there was potential to be hurt
carrying out the task and that a JSA should have been talked through and
signed before starting the work .

9 Audit of the drill floor working at height tool kit found that some of the DD Open .Suggestion
tools lanyards are starting to show their age, talk held with supervisor and
it was agreed that now the tool kit is 5 years old it's time for a new WAH
tool kit to be bought.

10 TOF's talk held at PTSM's and the crews were reminded about staying DD Closed .Suggestion
focused and not to become complacent , the work that has been carried
out and the work to be carried out over the next few days is very much
same same , the minute anyone loses concentration a incident will
occur .STAY FOCUSED.

11 Need to prep and paint air , water and saltwater supply lines for mud lab , DD Add to Work List .Tools & Equipment Unsafe
they are badly corroded and need some TLC , painters to action before Condition
logging unit comes back to the rig .

12 I observed smoke in the coffee shop I asked the Stc to check, found out PVDO Closed .Tools & Equipment Unsafe
welder was working near by, the Stc said good job for reporting the Condition
smoke .
13 Some of the grating on the walkway leading to the heli-deck is starting to HL JOC To be Investigated .Tools & Equipment Unsafe
rust, can it be painted to prevent it becoming worse . Condition

14 I came into the change room and the area was covered in smoke, I called ISOS Closed .Tools & Equipment Unsafe
some rig supervisors and they found that the smoke was coming through Condition
the drains from the welding job below the change room, the men then set
up air movers to ensure the change room stayed smoke free, the men's
quick actions were encouraging and worthy of note .

15 Water leaking in cabin toilet 206 and water leaking in cabin 303 , please PVD Logging Open .Tools & Equipment Unsafe
fix when time allows . Condition
16 I observed two yellow lights out on the heli-deck, I asked the elect dept to PVDO Open .Tools & Equipment Unsafe
action yet they have time . Condition
17 I observed the deck crew working safely as a team keeping the rig clean I HL JOC Positive Positive
asked the men to keep up the good housekeeping standards .

18 I observed the deck crew cleaning up the rig and placing all the different PVDO Positive Positive
waste into the correct waste skips and bins , good job .

19 Drill and deck crew working safely as one team laying down pipe , please PVDO Positive Positive
keep on working like that .
20 Deck crews working hard keeping the rig tidy , I said good job to the BJ Positive Positive
men .
21 Big thanks to the PSV guys for keeping the accomm clean and tidy, very MI Positive Positive
