Brief description of Observation cards Company Action Card Category

1 Man with his back to the V-door when equipment was coming onto the DD TOFS .Body Position
rig floor , TOF's talk held with the man about the importance of facing
moving equipment and having a planned escape route in mind and be
ready for unplanned movement of the equipment .

2 Man working a short distance off the ground with a number of tools in DD Closed .Body Position
his pockets, TOF's talk held with the man about the risk of him falling
and one of the screw-drivers puncturing his body, agreement to remove
the tools from his pockets .

3 Man picked up the slips out of the hole by himself, I held a safety talk DD Raise @ PTSM .Body Position
with him about the risk of him injuring his back, agreement to ask for
assistant in future .
4 Not all access points to task was closed off with barriers, TOF's called DD TOFS .Procedures
and the importance of keeping personnel away from the potential hazards
was talked through, to ensure this the men should have had all access
points to the work area closed , this was carried out before the job
continued .
5 Person observed handling a number of cups before deciding which cup he DD Raise @ PTSM .Procedures
would use, hygiene talk held with the man and he was asked not to
handle drinking items that other people would be using .

6 Making up dog-collar man was observed using a spanner wrench as DD TOFS .Tools & Equipment
hammer, TOF's called safety talk held about the risk of damaging the tool Incorrect Use
and more importantly a hand injury occurring due to the wrong tool for
the job being used.

7 When RIH I observed that the safety pin of the dog-collar was broken, I DD Closed .Tools & Equipment Unsafe
informed the driller and the problem was fixed . Condition

8 The air-point and water point next to where the logging unit normally DD Add to Work List .Tools & Equipment Unsafe
stands are very corroded, the pipework and fittings need to be changed Condition

9 The spare air valve of the brine line stbd side is broken, reported this to PVDO Open .Tools & Equipment Unsafe

10 Water spilling from toilet in cabin 307 can maintenance action when they PSV Catering Open .Tools & Equipment Unsafe

11 PSV men polishing the radio room floor , all required PPE was being PVDO Positive Positive
worn and safe working practices observed , good job .

12 Good work carried out by drill and deck crews when picking up drill-pipe DD Positive Positive
sideways , well done to all the men .
13 I observed the deck crew helping the elect dept in the stbd change room, PVDO Positive Positive
men working safely as one team , good job .

14 Good clear JSA prior to skidding operations , all men informed to call PVDO Positive Positive
TOF's at any time if they had a safety concern .

15 Good job by the deck crew making safe the CO2 locker, work completed PVDO Positive Positive
safely .
16 Good teamwork between the drill and deck crews when working with PVDO Positive Positive
drill- pipe, everyone looking after and helping each other , well done to
all involved .
17 Big thanks to the deck crew for helping the scaffolder, correct manual PVDO Positive Positive
handling observed and men were aware of their hand placement at all
times when handling the scaffold , good safe job .

18 Rig welder very busy working on new pipework in stbd lower machinery PVDO Positive Positive
space, all required paperwork was in place and correct PPE was being
worn , I said good job to him .
19 Good clear communication from motorman when he was involved in the BJ Positive Positive
transfer of fuel to the BJ unit, I thanked the man for his good work and
good comm's.
