 AOA & MOA with Incorporation certificate of the hospital (PVT.LTD)

 Company PAN card.
 TAN number
 Need to have 3 bank accounts ( 1.REVENUE 2. EXPENSE 3.TPA/Cashless)
 Scanned copy of all licences according to the city norms and regulations.
 ROHINI id and CMO licence for TPA Deptt.
 Pharmacist licence.
 Drug Licences by the name of company.
 Doctor and hospital indemnity policy.
 Fire/ Burglary insurance.
 10-15 supporting documents from the day of starting of the hospital for
TRADEMARK purpose.
 Any agreement document with pharmacy or opticals.
 If any legal case is going on in regard to the building/Hospital then all the
documents involved.
 Domain registered document.
 TPA mou documents.
 Telephone and electricity bills by the name of the company.
 Machines bills and AMC/CMC status.
 Last AMC/CMC done.
 Any pact with the machine provider for AMC/CMC.
Other Supervision aspects

 Marketing strategies online & offline.

 OPD and IPD files process.
 Counsellor – TPA- Doctor – Manager proper and clear cordination.
 Uniform and grooming of each staff.
 JD of each staff.
 TPA recovery process and status.
 Policies of hospital according to the city about holidays etc.
 Expenses process and structure. (max 1500 by petty cash if more than only
by bank)
 Make sure only GST paid bills are acquired and payments should be
through banks.
 KYC documents of Staff( Aadhar, Bank, Qualifications, PAN, Family details,
emergency contact)
 Anaesthetist process(Payrol or per case)
 Injections Schemes.

REPORTS

 OPD Report from April 2022 ( new+f/up+ Revenue) for each month.
 IPD Report from April 2022 ( No of IPD+ Revenue)
 INV Report monthly wise Revenue.
 Counselling Follow-up or non-converted patients data with full details
 IOL costing should be according to the decided amount as per the hospital.
 Advance and due amount reports with each detail and status.
