Sir / Madame:

This is to request permission from your good office to please allow the 2ND YEAR- HOTEL
OPERATION SERVICES students, to undergo a 200 hours of ON-THE-JOB-TRAIING in your
company/agency to acquire industrial ORIENTATION. The training period will start immediately upon
your acceptance of the bearer under the terms and conditions as stipulated in the duly accomplished
memorandum of agreement hereto attached for your confirmation.
Moreover, a student evaluation is provided where a student is to be assessed on his/her
performance, attitude and behavior to be attested by the supervisor. The On-the-Job Training
Coordinator and Trainer handling each field of specialization from our school will visit your
establishment from time to time to find out how the student perform in the working area. Our Teacher-
in-Charge will monitor the performance of the student if necessary, and that the immediate superior
within the company will take all the necessary precautions to keep the student safe.
However, if despite all efforts taken, untoward incidents beyond their control occur, we
will not hold your company/office and the West Central College of Arts and Science Inc. liable.

I. Narrative Report and Reflection

In this section, you are required to create a narrative of all events and activities that took place
within the indicated duration. This narrative, which is basically a summary of your whole term as
a trainee in this work immersion program, should give your individual coordinators a peak of
how you witnessed or experienced the term itself.

On the latter half of this narrative report, state the lessons that you learned for the entirety of
the work immersion program. You can share the struggles and difficulties you have encountered
during the job-hunting process and the completion of your requirements. In addition, you can
include challenges and problems encountered on site during your training period and how did
you cope with them. In this reflection, try to incorporate the school’s vision, mission, and core
values, as well as the objectives of this work immersion course as indicated by our Work
