Magrahat, District – South 24 Parganas, Pin - 743355, do hereby
solemnly affirm and declare as follows:

2. That the name of my father is PRABHAS CHANDRA BISWAS who

had died intestate on and the name of my mother is ARUNA
BISWAS who had died intestate on leaving behind their one son
as their only legal heirs and successors are as follows :-
