I, ______________________________, hereby acknowledge that I am participating

in a field research project titled “Loss of Livelihood: Estimating the Value of Pig Production
Loss Due to ASF in Smallholder Farms” as part of my Inquiries, Investigation, and
Immersion requirements for the School of CNCHS. I understand that this research involves
gathering data from farmers, and I agree to follow all necessary protocols and procedures
associated with this process.
I hereby confirm my voluntary participation in the field research project. It is my
intention to conduct this research and gather data outside the school. I acknowledge that the
School of CNCHS is NOT responsible for any consequences or risks that may arise during
my data collection process. This includes, but is not limited to, any physical harm, emotional
distress, or property damage that may occur while I am in the field. I agree to take all
necessary precautions and follow all safety guidelines provided by my supervisor and the
School of CNCHS.
I understand that I am responsible for obtaining informed consent from all participants
in the study, and I will adhere to all privacy and confidentiality requirements outlined by the
School of CNCHS. I agree to handle all data collected with care and confidentiality, and I
will not share any identifying information about participants without their consent.
I acknowledge that this research is being conducted for academic purposes only, and
any results or findings will be used solely for educational and research purposes. I understand
that the School of CNCHS is not responsible for any misuse or unintended consequences that
may arise from the dissemination of these results.
By signing this waiver, I confirm that I have read and understood the contents of this
document, and I agree to abide by all terms and conditions outlined herein.
