AGREEMENT

This Agreement is being made between Eugene P. Santos (As the Process Officer) Located at
Block 18 Lot 36, Mt. Sinai St., Pleasant Hills Subd., City of San Jose Del Monte, Bulacan and
Francis Junsay (As solar contractor) with client address at Block 3 Lot 26 San Lorenzo Heights
Phase 2, Dasmariñas City, Cavite.

Scope of Contractor:

• Professional services; includes fill-out forms to MERALCo Dasmariñas Business Center

and Office of the Building Official, Municipality of Dasmariñas, Cavite.
• Preparation of renewable energy supporting documents such as;
1. Certificate of renewable energy equipment
2. Data sheet of PV module and PV Inverter
3. RE Single Line Diagram
4. RE Plant Parameters
5. Filled-out application form interconnection of RE
6. Filled-out application form for ERC-COC
• Design of Solar Electrical layout
• Payment for Notarized Authorization Letter
• 5 sets of blueprint with sign and seal of Professional Electrical Engineer
• Payment for Difference in Meter Cost Fee
• Payment for Government Fee not more than Php 1,500
• Payment for Notary of Net Metering Agreement
• Payment for ERC Cheque

Scope of Owner:
• Submission of specified documents indicated as per checklist of MERALCo and Office of
the Building Official, Municipality of Dasmariñas, Cavite.

TOTAL PROCESSING COST: ₱26,000.XX
