HSE Fire Incident Caused by Mobile Phone


ALERT Charger

What happened?
During rig move, catering personnel observed fire in
an accommodation cabin and immediately
activated manual fire alarm. The fire team put on
SCBA and started extinguishing the fire. After putting
out the fire, one of the backup team members (IP)
was not wearing SCBA and inhaled fire extinguisher
powder.

Consequences:
● Property damage.
● Serious injury.

Why did it happen?

● Wrong practice, mobile phone was left unattended while charging.


● Substandard mobile phone charger.
● IP was not wearing SCBA.

What should be done to prevent reoccurrence?


● Do not leave battery powered devices unattended while charging.
● More frequent camp inspections shall be performed to verify that electrical
connections are up to standard and used correctly.
● All portable and extension cords shall be UL/FM/KEMA-KEUR approved.
● Stop work if employees are not wearing the appropriate PPE.

Areas where improvement is needed:


Workers must:
● Know and use the required PPE for a task.
● Stop work immediately if PPE is not available.
● Only use approved electrical cables.
● Never leave your phone unattended while charging.
● Understand lines of fire and follow precautions
associated with hazardous area.
Lifesaving Rule No. 8
Work supervisors must:
● Identify task-specific PPE.
● Confirm that workers are trained on the correct use of PPE.
● Confirm that all PPE required for a job is available and being
used properly.
