 Photocopy of Disbursement Voucher / Check for reference as to check number and
amount

3. BIR records (included names on transmitted documents to BIR regarding on sworn

declaration, registration fess payment and certificate of registration (COR)

4. BIR forms 2316 and 2307

 Completed request form (to be release on remittance window)
 Include a photocopy of valid ID
 Affidavit of loss
