SWP 01 Hot Work Permit

HOT WORK PERMIT


This check list must be completed prior to starting any hot work by verification of the MASTER. If any of the listed conditions
change, then this permit is invalid and a new permit shall be issued before work continues.

1 Have you conducted a Safety Meeting in accordance with SQEMS Manual Section 6 and completed Risk
Assessment Form in accordance with SQEMS Manual Sec 9 for the planned hot work and take the necessary
permission from Company?
2 Is the location of the hot work an enclosed space? NOTE: If Yes, the proper Enclosed Space entry Permit must be completed
and authorized.
3 Does the hot work involve any form of electrical maintenance work? NOTE: If Yes, the proper Electrical Maintenence
Work Permit must be completed and authorized.
4 Is the location of the hot work aloft , outboard or over the side? NOTE: If Yes, the proper Working aloft, Outboard and Over
the Side Permit must be completed and authorized.
5 Has the meter used to sample/monitor the welding area been calibrated in the last 24 hrs. (If not, do so)
6 Has the area to be worked on been thoroughly purged of hydrocarbons?
7 Has the area to be worked on been thoroughly ventilated?
8 Has the work area been cleaned by machine washing?
9 Have all of the surrounding areas also been machine washed and are gas free?
9a If not, are the surronding areas fully ballasted?
9b Are the surrounding areas inerted?
10 Can hot work be done safely?
11 If the work area is an oil pipe, has it been thoroughly flushed with water from both end and disconnected from the
surrounding piping?
12 Is the area free of all flammable debris and scale?
13 Have all potential sources of flammable vapors that could reach the site of the hot work been prevented
from going so?
14 Have ships drawings been referenced to ensure the area is safe for hot work?
15 To assure combustible gasses will not reach the work site, have wings and air current been considered?
16 Have bilges within the hot work area been inspected to assure that hot work can be done safely?
17 Have precautions been taken to ensure the personnel near the area are protected from the such hazards as fumes and ultra-
violet light?
18 Have personnel been provided with proper Personnel Protective Equipment (PPE)?
19 Has a fire watch been set up with appropriate fire fighting equipment?
20 Can the fire watch continually see the worker? (If not, a safety watch must be set for this purpose)
21 Have the workers done a job Hazard Analysis in which clear instructions have been given on how to minimize circumvent
these hazards?
22 Have instructions and contingency plans have been discussed in the event of an emergency?

23 If welding,the earthing connection is adjacent to the work site and earth return cable leads directly back to the welding
machine. The ship's structure should not be used as an earth return
24 Has the equipment to be used been inspected prior to starting work and found to be in good condition?
25 Is the fire main charged, hoses let out, monitors pointed out and all in good conditions?
IF ANY CHECK MARKS MUST BE PLACED IN THE SHADED AREAS, THEN THIS ITEM MUST BE
RECTIFIED BEFORE PROCEEDING

Atmosphere should be continously monitored with personnal gas meters. Additionally, officer in charge of safety should check
and record the atmosphere test results with intervals not exceeding 20 minutes. (See Note 2)

Notes:
1. Immediately before hot work is started the officer responsible for safety precautions should examine the area where hot work is to be
undertaken, and ensure that the oxygen content is 21% by volume and that tests with a combustible gas indicator show not more than
1% LFL
2. Officer in charge of the work should ensure that atmosphere is continously monitoring by personnal gas meters. In addition, officer in
charge of safety should re-test the atmosphere after each break in work periods and also record the test results of atmosphere at regular
intervals which should not exceed 20 minutes.
3. If any other Permits are required, Hot Work Permit should be the last permit issued.
4. A Hot Work Permit can only cover a single space. A seperate permit should be approved for each intended task and location.
