(5) thereof, I do hereby appoint you as Barangay Health Worker of Barangay
Bonifacio, San Carlos City (Pangasinan) to take effect immediately upon approval by
a majority of all members of the Sangguniang Barangay, this barangay, and after
taking your oath of office before any authorized administering office.

You shall therefore perform the duties and functions of your position and
enjoy the benefits and privilege appurtenant thereto as provided by the law and
issuances but not beyond the expiration of my term of office unless sooner
removed or suspended for cause.

WORKERS OF BARANGAY BONIFACIO, SAN CARLOS CITY (PANGASINAN)

WHEREAS, pursuant to Section 389 (b) (5) of Republic Act No. 7160 otherwise known as
the Local Government Code of 1991, Punong Barangay Alex G. Martinez appointed the following
bona-fide resident of Barangay Bonifacio, San Carlos City (Pangasinan) to serve as Barangay
Health Workers thereat;

and duly seconded by all the Sangguniang Barangay Members present;

RESOLVED, AS IT IS HEREBY RESOLVED, that the appointments of the listed Barangay

health Workers be approved and concurred by the Sangguniang Barangay Members of Barangay
Bonifacio, San Carlos City (Pangasinan).

RESOLVED FINALLY, that copies of this resolution be furnished to all concerned offices.
