HAZARD IDENTIFICATION/RISK MANAGEMENT TEMPLATE – HLTWHS001

INSTRUCTIONS: How to complete this table?


 Identify the potential hazards that exist in the area you have been asked to inspect or concerning the planning activity.
 Identify the potential impacts that could be caused by exposure to the hazard. You should consider a) How the harm could occur, b) The potential harm, and c) Who is at risk of
exposure.
 Determine whether the hazard you have identified is removable and if any existing controls are in place.
 Complete an assessment of each hazard's risk by analysing both the likelihood and the consequence of exposure, and then rate the risk using the risk rating matrix.
 Recommend a treatment for the risk (i.e., how to eliminate or reduce the risk for participants).
 Nominate a duty holder/person/group who is responsible for implementing the treatment that you have recommended.
