Regarding confirmation of non-receipt of uniform allowance

I would like to inform you first hand that I had been working as Public Health Inspector in
Trincomalee District General Hospital from 1st of 6th month of 2022 to 15th of 2nd month of
2024. Also, on 15th of the second month of 2024, I transferred and assumed my duties in the
office of the Medical Officer of Health, padaviSripura.

I have to receive my uniform allowance for the year 2024 from Trincomalee Regional
Director of Health Services.

I have been asked by the Accountant of Trincomalee Regional Director of Health Services to
confirm that I have not received the said allowance from your trincomalee District General
Hospital.so please confirm the above matter.
