Saipem Classification - General Use


INTRODUCTION

1.1 OBJECTIVES OF THE DOCUMENT


The scope of this document is to define the general criteria in order to define and
perform the medical examinations that all Saipem Group’s employees shall undergo
before their employment and during the working retainership.

The purpose is to:


• determine whether a potential employee or an employee is medically fit to
carry out specific duties or to operate, in particular weather and environmental
conditions.
• conforming to his/her health conditions.
• monitoring and amending eventual impairment to the employees’ health.

1.2 AREA OF APPLICATION


This Criteria applies to all SC/VENDORS/TIERS/PARTNERS, CT in T7 LNG Bony Island
Project.
Find below the description (checklist) of MFC Protocol C.

MFC PROTOCOL "C" Checklist


(All Following Medical test MUST be present to complete protocol)

INSTRUMENTAL EXAMINATIONS
General Medical Examination
Chest X-Ray
ECG
Audiogram
Ophthalmological Exam
Spirometry
Digital Pulse Oximetry
Ethylometry

LABORATORY EXAMINATIONS

Full Blood Count (FBC)


Blood Sugar (glycemia) in mg/dl
Blood Urea Nitrogen (BUN)
Transaminase (AST/ALT)
Gamma G.T.
Bilirubin (Total and Direct)
Cholesterol Total in mg/dl
HDL / LDL in mg/dl
Triglycerides in mg/dl
Urine Examination (Physical, Chemical & Microscopic)
T.P.H.A (V.D.R.L)
Markers Hepatitis A, B and C
HIV

Drug Test in Urine, Shall contain (as minimum) the following tests:
a) Amphetamine
b) Benzodiazepine
c) Cocaine
d) Marijuana
e) Methamphetamine
f) Opiates

Remarks:

• If any of mentioned test is missing, the personnel will not be cleared by medical
department for proceed his/her process.

• Date of Validity of MFC Document Issue date and Expiration Date MUST be
present in front page, using this format DD/MM/YY e.g 14/12/23.

• Every MFC must be signed and stamped by a license Doctor, full name of doctor
required, no abbreviations.

• The maximum validity of the Health Certificate shall be two years (24 months),
starting from the date on which the medical examination is performed.

A validity period shorter than 24 months shall be accepted based on the following.
conditions:

• When it is specified by the examining physician due to health surveillance


limitations or restrictions.
• When it requires contractually or by the local laws and regulations.
• When it requires by the specificity of professional role or task.

T7 Medical Dept can request additional test not listed based on


Health Risk assessment in project or health condition.

ANNEX:

PROTOCOL G (ONLY FOR CTAERING FOOD HANDLERS)


Complete full MFC Protocol C above, plus follow tests:

• Throat swab test


• V.D.R.L
• HAs Ag
• HAVAb
• Stool Examination for parasites
• Food Handler Course Certification (6 months validity)
Remarks: MFC Protocol G, must renew every 12 months.

Other requirements only for EXPAT personnel on
mobilization phase which are mandatory according to
Nigerian Regulations:
• Yellow Fever Vaccination
• COVID-19 Full Vaccination Scheme

If not fully vaccinated or unvaccinated at first arrival to Nigeria, find below protocol.

COMPLETION OF HEALTH DECLARATION FORM: All international travelers MUST register


via the Nigeria International Travel Portal (NITP– https://nitp.ncdc.gov.ng) and proceed to fill
in the online Health Declaration/Self-Reporting form located on the portal. They should ensure
that the information/ contact details provided on the form are correct and verifiable. Travelers
must provide valid phone numbers, residential addresses, and functional e-mails where they
can be contacted. Travelers must also inform Port Health officials on arrival of any change in
their health status since completion of the Health Declaration/Self-Reporting Form.

Once the Protocol is completed, send the complete list of staff member/members using below
excel form attached. (NLNGT7-BOC-VEN-GEN-0014 -MFCs DATA BASE) (find image below for
guidance). Click on image to go file.

MFC Protocol scans must be in PDF single file for each worker following below naming system:

MFC_FULL NAME.pdf
