Dear Sir/Madam,
APPLICATION FOR HEALTH FINANCIAL SUPPORT TO CATER FOR MY HEALTH.
I am I the person Mr. Daniel Abugre Abugbire with staff Id : 838702, a registered nurse of Ghana Health
Service and a Staff in the Bongo District hospital and an active member of the Association from 2015 till
date.
However, I have been sick since the middle of last year and to make matters worse I issues with my salary
in October,2023 due to non- validation and I have to battle with both financial, health and psychologically.
As I was trying to find out reasons for the non-validation, my current post (Bongo District Hospital) secretary
approached me through inquiry from the Regional Health Directorate that I have been transferred from
Bolga Nurses Training College to Bongo District Hospital without my knowledge or transferred letter and
that prompted me to find out the school about the transferred and they said they affirmed the knowledge of
it. I went to my newly posted hospital and met matron and to eventually report but luck eluded me I was
admitted at the Upper East Hospital the next day for several weeks later referred to the Korle-bu Teaching
Hospital but due to the financial challenges I was unable to go, my wife and family members have to take
loans because I was bedridden and finally transported me to Korle-bu Teaching Hospital.
I wish to humbly request for your kind support on the above subject to assist me cater for my medical bills
because my wife and family members have exhausted every available means.
I hope my appeal will be kindly considered.
