Transference-Focused
Psychotherapy (TFP) and Its
Applications: Introduction
to TFP and the Roles
of the Structural Interview,
Goal Setting, and Contracting
1.1 Introduction
Transference-focused psychotherapy (TFP) is a specific
treatment developed over many years for a specific patient
population – individuals with borderline personality disorder
(BPD). TFP is a manualized treatment (meaning clinicians
are guided by a detailed treatment manual outlining the
approach to treatment) and evidence-based (meaning it has
been studied and compared to other treatment interventions
for the same patient population). TFP has been shown to be
effective in multiple domains: retention in treatment (patients
staying engaged in psychotherapy vs. dropping out early),
suicidal and self-injurious behavior (including the number of
self-harm attempts and the medical severity of these
attempts), self-reports of symptoms (a reduction of suicidal
ideation, depression, and anxiety), and social adjustment
(positive changes in friendship and work) [1–3]. This accumu-
lated data supports TFP as an individual psychotherapy for a
particular patient population; numerous studies and textbooks
have been published on this subject. The goal of this book is
to further extend the influence of TFP theory and practice by
introducing TFP as a way of thinking about and treating
patients beyond a limited group (patients with BPD by DSM-5
criteria) and beyond a particular treatment modality (extended
individual psychotherapy) for which it was developed.
Who are the clinicians who can use TFP principles in their
work, and what are the clinical situations where TFP princi-
ples can be useful in patient care? The answer to this question
begins with consideration of who the clinicians are in the
mental health and medical fields treating patients with sig-
nificant personality disorder (PD) pathology. The answer is
almost all clinicians seeing adolescent and adult patients. Are
there exceptions? To be sure, clinicians treating only young
children, or those patients with primary psychotic disorders
or dementia, for example, would be in this category. That said,
patients with PD pathology are well represented in almost all
mental health settings and in many general medical settings

informing consideration about a patient’s level of
personality organization and prognosis.
3. The clinician’s active monitoring of three channels of
communication: what the patient says, how the
patients acts, and how the therapist feels.
4. Appreciation of the admixture of repression-based
defenses and splitting-based defenses a patient
exhibits.
5. Elucidation of a patient’s personal goals and treat-
ment goals.
6. An extended process of contracting, leading to the
establishment of a living treatment frame, tailored to
the specific pathology of, and expected challenges
from, the individual patient.

9. The therapist’s efforts to maintain a stance of con-
cern that reflects technical neutrality in relation to
the internal conflicts the patient struggles with, with
deviations from this stance when required.
10. Ongoing attention to the treatment frame, with
periodic adjustments to the frame as necessary,
given likely challenges from the patient.

experience of the therapist (and others in the
patient’s life) with increasing trust.
19. The therapist’s appreciation of the interplay
between dyads (relationship paradigms carrying dif-
ferent emotions).
20. The patient’s gradual move from the paranoid posi-
tion (projection of negative emotions) to the depres-
sive position (taking ownership of negative emotions
with the capacity for feelings of guilt) reflecting
integration of previously “split” representations of
self and other as totally good or bad.
