Dear Paul,

Re: Haseeb Mahmood, 72 Southfield Square, Bradford, West Yorkshire, BD8 7SN
Student Operating Dept Practitioner

Thank you for referring Haseeb Mahmood whom I assessed via a telephone consultation on 27th March 2024.

Haseeb was counselled about the nature of the consultation and has given verbal consent to provide you this
report about his health and fitness to work. He will receive a report at the same time.

Background
He reports history of stress and anxiety due to personal related matters. He reports a close relative is
receiving treatment for a medical condition and this situation has impacted him. I understand the individual is
doing well and is well supported.

Current Situation
He reports he has started counselling and is also using the coping mechanisms learnt from previous cognitive
behavioural therapy and is feeling better in himself. He states he is sleeping well, his mood has improved, and
his appetite is improving. He has also been in touch with the wellbeing team at the University and is on the
waiting list for counselling.

Answer to Specific Question

Q1. Is Haseeb fit to return to placement in an Operating Theatre?

A1. Haseeb seems to be heading in the right direction as he reports improvement in his symptoms, he has
insight and has sort appropriate support, also using previously learnt coping mechanisms. On these bases, I
find him fit to return to placement.

His preference is to work night shifts as he finds he is calmer and more relaxed. I don’t know how feasible this
is during placement, but I will suggest you look into this as a possible adjustment.

He will benefit from periodic one to one support from his tutor/mentor to assess how he is coping, identify
early signs of stress and where adjustments might be required.

Review
None planned.
With kind regards,

Yours sincerely
