Valve Failure Analysis Report

General Instructions
1. Photograph anything that might help us understand
the nature of the failure.
2. Remove the failed valves, and contact Ariel prior to
disassembling.
3. Complete this electronically fillable report. Complete
a separate report for each type of valve failure. Sim-
ilar valve failures may be submitted with a single
report.
4. Package the valves carefully and contact Ariel Tech-
nical Service to request an RMA number. Include all
photographs and this report with the valves, or email
photographs and report to
fieldservice@arielcorp.com.
