AFFIDAVIT OF UNDERTAKING

Palawan____________________________, after having been duly sworn to in accordance with law, do hereby
depose and state that:

I am a bonafide member of the Philippine National Police presently assigned at ______401st B Mc

RMFB 4B________________________ and currently as a participant of _______PSJLC____________ with RTC
MIMAROPA through blended learning modality;

I am fully aware and shall faithfully abide of the policies, rules, and regulations governing the
training stipulated under the NPTI Training Guide (Revised 2018) which is in effect whether inside or
outside the premises of the Training Center;

I declare my residential and office address below as the authorized training venue while attending
virtual classes and/or during the whole duration of the course;

I am fully aware that in case of necessity to leave the aforementioned addresses due to
valid/emergency reason/s, the policy on pass as stipulated in the Training Guide (Section 8-6) must be
observed and that violation thereof must be dealt with accordingly;

I undertake to faithfully abide by the Classroom Courtesy and Decorum during Virtual/Synchronous
Instruction set forth in the said Training Guide (Section 5-5);

I undertake to report by all means to my Course Coordinator/Training Officer and other members
of the training staff any untoward incidents or emergency situations I will be involved in while on
schooling;

I undertake to promptly report to the proper authorities of this Training Center any health/medical
issues I may suffer while inside or outside the Camp for further medical assessment and/or to do the
same if this is also noticed from other colleagues;

I fully understand that I am under the disciplinary authority of the Training Center within the
duration of the training period both inside and outside the Center’s premises and that violation of any of
the foregoing provisions is a ground for disciplinary actions against me pursuant to NPTI Training Guide
(Revised 2018); and

I execute this affidavit to attest the truth of the foregoing and for whatever lawful purpose it may
serve.
