Brief description of the lifting operation: Lifting of the Winch frame (No.1 and 2) and loading it to the truck

Method statement step by step instructions:

1. All personnel associated with lift should review Risk Assessment and complete TBT prior to commencing
lifting operation.

2. Banksman, crane operators to ascertain environmental conditions are within crane parameters before
commencing lifting operation.

3. Lifting crew ensure no conflicting activities are ongoing in surrounding/landing areas and /load path is clear
of obstructions.

4. Lifting crew ensures barriers have been placed covering all intended load path during lift.

5. Lifting crew to carry out final checks to ensure no dropped object hazards such as tools and other equipment
have been accidently left on the load.

6. Mobile Crane operator to set up Crane at nominated area at max radius of 6 mtr.

7. EOTC Operator bring Overhead Crane above the frame in dedicated area.

8. Install all accessories as shown on the picture (2 leg wire rope sling with 25 Te shackle on each leg should be
installed on the padeyes near the winch, two 8 Te round slings should be installed on other end padeyes by 25
te shackle, top end attached by 35 Te shackle to the load cell on EOTC hook)

9. Banksman give signals to crane operator to bring the hook above the load.

10. Riggers attach two tag lines to the frame.

11. Under the instruction of the banksman position crane boom over the load (max boom length is 11.5mtr) and
ensure load line is plumb.

12. Truck should park in line with the load in a distance of about 2 meters.

13. Banksman check that Crane hooks alignment is in line. Banksman to instruct Crane operators to slowly take
the tension, slowly take weight of load and stop and ensure the weight of load is proper on the both cranes
(check the load on EOTC with the load cell – load should be no more than 7.88 Te) and slings are not twisted.

14. Lift the load at maximum 1.5mtr height from the ground and stop.

15. Banksman give the signal to the truck driver to move under the load.

16. Once in place, Banksman ensure truck driver is away from truck and load on a safe area.

17. Banksman confirm the landing point and give signal to lower down slowly.

18. As soon as load is landed to the bed of the truck Banksman check correct position and security of the load.

19. Banksman gives signals to give a slack to the slings.

20. Riggers disconnect the slings from the hooks and load.

21. Fold up the crane.

22. Riggers inspect the slings and clean the area.

23. Barrier removal.

24. Above mentioned steps to be used for Lifting both Winch frames. Crane will be repositioned
and all parameters will stay same as mentioned in LP.

De-brief and Lessons Learning:


Location of the Winch No.1 was changed to another workshop where the height of the roof is 10.2mtr max.
To gain more headroom was agreed to use smaller crane: XCMG QY70K-1.
Crane was tested prior the operation by AMOS.
Lifting capacity of the crane on dedicated radius is 38.5 Te @ 6mtr.
Utilization factor changed from 55% to 60.3%.
