DISCHARGE PERMIT REQUIREMENTS

New Application
1. Duly accomplished and notarized application form (downloaded from OPMS site).
2. Engineer’s Report prepared and signed by Chemical or Sanitary Engineer and Pollution Control Officer duly
endorsed by the Managing Head
 Nature of the project or business to include description of processes including the production capacity
(quantity or volume) and generic name of the product
 Nature and characteristics of wastewater (physical and chemical composition) and total daily volume of
discharge of raw wastewater
 Treatment process and estimated treatment efficiency
 Total daily volume of water consumption and final discharge of final treated wastewater or effluent
 Information on flow measurement equipment and procedure
 Name of receiving body of water and its official classification
 For land discharges, the nearest receiving body of water and its official classification
 Pollution prevention/Environmental management System pan or program
 Statement of the cost incurred in the installation and maintenance of the wastewater treatment facility
 Quality and quantity of the abstracted water
3. Vicinity map identifying the street address, location of the plant premises
4. Plant Layout and specifications duly signed and sealed by the Chemical or Sanitary Engineer and Pollution
Control Officer
 Plans and specifications of the proposed Wastewater Treatment Facility (WTF)
(For Septic Tank, include septic tank computation)
 Wastewater Line Layout
5. Latest Result of Analysis of Effluent collected and analyzed by a third party EMB Recognized Laboratory or
Affidavit of No Discharge
6. PCO designation signed by the Managing Head
7. ECC or CNC
8. Penalties/fines and/or arrears paid
9. Proof of Compliance with RA 9003 (ESWM ACT)
 Photo of designated temporary storage area/MRF for recyclables, electronic wastes, BFLs, batteries and other
special wastes.
 Photo of backyard composting area/ facility (if applicable) or proof of collection of
compostable/biodegradable wastes.
 Copy of Contract/MOA with the LGU as hauler and with Contract/MOA with accredited Sanitary Landfill
(SLF)/Co-Processing/TSD Facility operators for disposal of residual wastes.
 Copy of Certificate of Disposal/Treatment issued by accredited SLF and TSD Facility operators
10. Proof of Compliance with RA 9275 (Clean Water Act) and/or Supreme Court Mandamus to Rehabilitate Manila
Bay
 Copy of Certificate of siphoning/desludging of septic tanks, hauling, and transport and treatment of
wastewater/septage by an accredited service provider
11. Copy of Tenure Instrument (e.g. Original Certificate Title, TCT, CLOA, Deed of Absolute Sale, Contract Lease
Agreement, FLAg, FLAgT, FLA, SPA, SIMA, MPSA, Area Status/Clearance or WLA)
12. Water billing statement for the whole year
13. Copy of official receipt of the required fees (Order of payment will be downloaded from the OPMS site)
a. Application Fee (Filing Fee, PD1856 & Documentary Stamp Tax) (Step 5)
b. Load Based Fee (Permit Fee and Waste Water Discharge Fee)
14. Certification from the LGU – Engineering/Sanitation Office that they inspected your Septic Tank and that it is in
accordance to the requirements of the Code of Sanitation of the Philippines (PD856) and its IRR.
15. National Water Resources Board (NWRB) permit for deep well or free flowing (direct connection) source of
water.

Renewal Application
1. Duly accomplished and notarized application form (downloaded from OPMS site).
2. Copy of Ambient Water Quality Monitoring Results (if required)
 Please refer to ECC and DP conditions
3. Latest Result of Analysis of Effluent collected and analyzed two (2) months before the expiry date
by a third party EMB Recognized Laboratory
4. Certificate of PCO Accreditation
5. Registration Certificate as Hazardous Waste Generator/s
6. Self Monitoring Reports submission with attached laboratory results of effluent (1 st, 2nd, 3rd and 4th
Quarter) Compliance Monitoring Report (CMR) and CMVR (if applicable)
7. Recent Discharge Permit issued by this Office.
 Issued with NOV/Order
 Proof of Completion/undertaking commitments made during technical meeting/s
8. Penalties/fines and/or arrears paid
9. Proof of compliance with DP conditions
10. Proof of Compliance with RA 9003 (ESWM ACT)
 Photo of designated temporary storage area/MRF for recyclables, electronic wastes, BFLs,
batteries and other special wastes.
 Photo of backyard composting area/ facility (if applicable) or proof of collection of
