Types and Classifications of Accident Investigations:


When an accident/ near miss/ MVA (Motor Vehicle Accident) is reported, below are the
escalation Routes that need to be followed.

Employees Responsibility is…


1) If required, seek appropriate First Aid at the site you are on.
2) Inform your line manager/ Member of the Senior Management Team of the incident
immediately and the customer if on a customer site.
3) Preserve the scene of the incident and take pictorial evidence, to aid the future
investigation.
Line Managers Responsibility is….
1) Escalate the incident to the appropriate persons within the organisation.
2) Ensure the wellbeing of the injured party after the incident.
3) Obtain the first aid record from the first aider - xxx
4) Obtain a Witness Statement, can use the following form xxx
5) Start the Accident investigation process using xxx
6) Aid the identification of an immediate/ permanent corrective actions and implement
them.
HSE Professionals Responsibility is…..
1) To upload event into the Accident Investigation system (HS System) and HSE
System.
2) Aid the Investigation process and ensure a RC has been determined.
3) Facilitate the formulation and completion of immediate and permanent corrective
actions.
