SUBJECT : Final Report re Traffic Incident

1. Reference: Progress report from IO, Pitogo MPS dated March 15, 2020 with
subject same as above.

1. March 15, 2020, 2:15 PM, Purok 1, Barangay Liguac, Pitogo, Zamboanga del
Sur.

2. A vehicle traffic accident occurred involving an Elf Truck due to mechanical brake
failure, which resulted in the death of eight laborers and injuries to eight others. The
truck was loaded with huge timbers, and the driver, Leolito Tam Gupio, was identified
as the cause of the accident.
3. The accident took place when the driver failed to control the truck, leading to a
collision with several workers. The impact caused fatalities and injuries among the
laborers present at the site.

4. The truck driver, Leolito Tam Gupio, was brought to Pitogo MPS for proper
disposition, and the necessary legal actions were initiated. IOC PSMS Christopher R.
Agan is handling the case.

5. It is recommended that appropriate charges of Reckless Imprudence resulting in

Multiple Homicide and Multiple Physical Injuries be filed against the truck driver,
Leolito Tam Gupio.

6. The case is ongoing, and the charges are being prepared for filing in court. The driver
will face legal consequences for his negligence, which led to the tragic incident.
