residence at 22 Gumamela St Longos, Malabon City and 32 Gumamela St Longos, Malabon City.

 That we are friends and long time neighbors of VIRGILIO OBISPO FLORENCIO;

 That upon securing a copy of his records from the office of the Philippine Statistics
Administration and Civil Registrar of Municapality of Aklan;

 That it was discovered that he has no transcription from the Register of Births or a true
copy of the Live birth;

 That we executed this affidavit to attest the truth of the forgoing facts and for whatever
legal purposes this may serve.
