PATIENT RIGHTS

I. Access to care.
2. Avail treatment & respectful care in a safe environment.
3. Free from any form of physical, verbal abuse or harassment from the hospital authorities or
staff members.
4. Access to all relevant information regarding the disease, course of treatment, discharge
planning & cost of treatment.
5. Continuity of care.
6. Respect all cultural & religious beliefs
7. Respect for privacy to ensure confidentiality.
8. Access to his / her own medical records.
9. Consent or reject any treatment lines, research activities or clinical trial offered.
10. Informed consent before any invasive/high risk procedures treatment.
11. Raise complaints and grievances, if any.
12. Right to voice complaint or grievance to a higher authority in the health care organization and insist
in writing on the outcome of the complaint

PATIENT & FAMILY RESPONSIBILITIES


I. Provide information regarding the patient's illness or any changes in the patient’s condition.
2. Provide correct and truthful history.
3. Provide complete and accurate personal information (full name, address, other information).
4. Be on time in case of appointments.
5. Inform hospital authorities well in advance in case of cancellation or rescheduling of appointments.
6. Ask questions to your physician or other care providers when you do not understand any information or
instructions.
7. Not to give medication prescribed for him/her to others.
8. Take care of hospitals property and its premises.
9. Accept, where applicable, adaptation to the environment to ensure a safe & secure stay in hospital.
10. Provide the surrogate decision maker if needed.
11. Comply with the facility policies & procedures, including those regarding smoking, noise and visitors.
12. Give respect & consideration to all hospital staff.
13. Respect that some other patients medical condition may be more urgent than yours and accept
that your doctor may need to attend them first.
14. Inform the doctors and administrative staff well in advance for the documents required for
reimbursement to hospital expense by a third party.
15. Pay for the services as per hospital rules.
16. Follow the treatment plan and carefully comply with tho instructions given.
17. If follow up treatment course suggested, take prior appointment
18. Understand the charter of rights & seek clarification, if any.
19. In case rights are not honored, inform the authorities.
20. Follow all rules and regulations of the hospital.
