Subject: ENQUIRY AGAINST SYED MUHAMMAD ALI SHAH TSV SHAH FAISAL COLONY

ENQUIRY MATTER:

Complaint received from Dr Sidra Azam regarding Negligence, Lack of interest, misconduct
and unprofessional behavior vide letter No THO/SFTK/14/3 dated 02/04/2024.

Enquiry was conducted by Dr Akhtar Abbas Additional DHO preventive along with Dr Waqar Farooqi
Assistant DHO preventive/DFP.

FACTS AND FINDINGS:

In the context of subject above, details of Mr. Syed Muhammad Ali Shah (on rendered service 89 days) is
as under: -

1. Relieved by DHO West due to non-punctuality vide letter No. DHO/WEST/584/89

dated 29-02-2024.
2. Joined EPI Directorate on dated 01-03-2024.
3. Placed at the disposal of DHO Korangi and to kept under strict observation and
monitoring in terms of EPI work vide letter No. PDEPISK/E-I & II/4405/17 dated 05-
03-2024.
4. Non-compliance report issued from DHO Korangi vide letter No. DHO/DKK/2758/62
dated 14-03-2024.
5. Assigned to work as TSV Shah Faisal Town vide letter No. DHO/DKK/3111/13 dated
26-03-2024.
6. Failed to report duty with effect from above to till date without any intimation or
prior permission to competent authority.
7. Received complaint from Dr Sidra Azam Town Focal Person Shah Faisal Colony
reflecting negligence, Lack of interest, misconduct and unprofessional behavior vide
letter No THO/SFTK/14/3 dated 02/04/2024.
8. Refused to join duty during meeting with TFP Shah Faisal Town.
9. Exhibited unprofessional behavior and disrespectful dialogue with TFP.
10. Showed lack of interest in department and unwillingness to work.
11. Committee verified said incidence and also found him physically absent from duty.

RECOMMENDATION:

1. On the basis of above grounds, Mr. Syed Muhammad Ali Shah has found absconder,
it is therefore recommended to relieve him from District Korangi to report EPI
Directorate
2. In the said context, mater may be forward to higher authority for further disciplinary
action against him.
