Reason and Source of Referral

Due to continuous increase in psychological problems her family doctor referred her to
psychiatry of BVH Bahawalpur.

" ‫ تاریخ کو الٹیاں شروع کر دیں۔ اس حالت خراب ہو‬2 ‫ پچھلے مہینے میں جب اس نے‬، ‫مہینہ پہلے یہ ٹھیک تھی‬
‫گئ تھیاور وہ بے ہوش ہونےکو تھی۔ اسے بے چینی تھی ۔ عالمات کیونکہ اس کے پاس بورڈ کے امتحانات تھے لیکن‬
‫وہ بیمار تھی اور اچھی طرح سے پڑھائی نہیں کرتی تھی۔ وہ امتحانات کے لیے تیار نہیں تھی۔ وہ قے کے عالج کے‬
‫لیے ہسپتال میں تھی اور ٹھیک سے پڑھائی نہیں کر پا رہی تھی۔ وہ ہسپتال سے ڈسچارج ہوئی اور سیدھے بورڈ کے‬
‫ نیند میں اضافہ اور غنودگی کی شکایت کر رہی تھی۔ وہ امتحانات کے نتائج کے بارے‬،‫امتحانات میں گئی۔ وہ سر درد‬
‫میں فکر مند ہے کیونکہ اس نے کہا کہ اس کے امتحانات اچھے نہیںں ہوئے تھے۔ اور اسے دوائیوں کی وجہ سے‬
‫"غنودگی ہے۔‬

She was alright 1 month when she started vomiting every 2nd or 3rd day after. In that situation

she was having episodes of loss of consciousness. She has anxiety symptoms because she has

board papers but she was ill and does not study well. She was not prepared for exams. She was in

the hospital for the treatment of vomiting and can’t study properly. She discharged from hospital

and directly went to board exams. She was complaining about headache, increased sleep and

drowsiness. She has anxiety about the result of exams because she said her exams were not well

performed. She has drug induced drowsiness.
