You should aim to complete this section in 100 - 200 words.

Note: this section can be completed after you have completed your risk assessment.
I did some preliminary research before entering into the factory. I went through the website of HSE
Outline how the risk assessment was (https://www.hse.gov.uk/) and ILO (https://www.ilo.org/global/lang--en/index.htm) for getting knowledge
carried out this should include: about different hazards, health and psychological effects and control measures to decrease or eliminate these
● sources of information consulted; hazards. Then I studied national and company law and regulations.
● who you spoke to; and
● how you identified: Through the work area inspections, I walked around the entire work site, reviewing workers, supervisors,
- the hazards;
engineers about their work and hazards in the workplace. It was realised that most of the technicians did
- what is already being done; and
not have the necessary awareness about the safe system of work, hazards and risks involved; they did not
- any additional controls/actions
receive any proper training.
that may be required.

I visited the working places and observed the various activities from different sections by visual inspection
and noted some hazardous activities. Also I suggested some control measures to those activities. I checked
company policy documents and got sufficient information regarding their safety culture. I also go through
previous accidents, incidents, first-aid causes and near miss records. Later, I interacted with the Managing
Director and Managers of respective sections from which I got required information. Afterwards, I have
keenly verified insurance policy details and some third party documents.

I checked personal protective equipment availability and also inspected whether the workers are wearing it
properly. By collecting this information from a workers representative, I understood the basic requirements
for workers.

After visual inspection and monitoring, I reviewed external information sources such as their health and
safety policy, safety plan & procedure, accident history, first-aid registers, sickness register, health
records, absence records, previous method of statement & risk assessment records, inspection reports,
and audit records. For more information, I site information from RRC (IG2) text books and a few safety
websites regarding health and safety; such as
https://www.ilo.org/global/standards/internationallabour-standards-policy/lang--en/index.htm,
https://www.hse.gov.uk/pubns/books/hsg150.htm Also take the relevant information from the senior
managers and the consultant managers.

Moral, general legal and financial arguments for ALL actions

Moral, general legal and financial arguments Sulzer limited has a moral commitment towards its workers to protect them from any harm or injury while
working for the organisation. Workers come to work to make wages for a living and they do not expect the ill
or injured will do tasks for the organisation. When a worker gets injured due to a major accident, it not only
affects him but also affects his family and friends. Long term illness or injury will affect the mental health of
not only the injured worker but also people dependent on him. The mental health of the co-workers might also
get affected if they happen to see a major accident.

Sulzer limited has legal requirements to protect its workers under the ILO’s Safety and Health Convention
(C155). The organisation is following health and safety policy that is built upon national and international
laws. If the organisation doesn't take any action to avoid workplace hazards that will lead to cease work.
Workers must provide a safe working environment according to the Conventions and Recommendations of
the International Labour Organisation (ILO). The violation of health and safety policy is a serious crime. The
head of the organisation or employee who is responsible for the incautious action may get fined or go behind
bars.

The financial burden Sulzer limited has to face are, Costs associated with injured workers such as medical
treatment, substitute worker wages, first aid cost, lost productivity time, medical sick pay, etc. The cost
associated with replacement of damaged equipment, structures, infrastructure improvement etc. Cost due to
Enforcement agencies. The organisation has to face enforcement actions such as improvement notices,

stopping of particular works, issuing simple cautions, service notices prohibition etc. If the organisation is
not able to continue these works, it will have a great effect on the finances of the organisation. The workers
who have left the organisation long before can give compensation claims against them due to illness or
permanent disabilities caused during their work activities for the organisation. All these claims can bear a
huge amount and will push the company to great financial loss. Additional costs such as lawyer fees, court
fines etc will also be present. The insurance company will not cover this cost and will lead to a financial
debacle for the company. If any major accident occurs in the organisation, Sulzer limited will lose its
reputation in the public and will lead to the cancellation of the current project and also future projects. These
will cause a huge financial loss and may also cause the closure of the company

Justification for action 1

Action
Construct separate storage area to store the flammable chemical in a separate area from the equipments and
machineries (hazard category fire):
Specific legal arguments
As per the ILO C167 safety and health in construction convention, 1988. (NO.167),at work site all
defence should be taken in case of fire.In every workplace of construction site, all applicable safety measures
shall be taken to prevent epidemic of fire and it’s spreading inside of construction area, both internally and
extremely, and to provide and maintain safe means of escape for all persons in the occurrence of fire.

According to the ILO C155 – occupational safety and health convention, 1981 (no. 155), ILO
R164-occupational safety and health recommendations, 1981 (no 164), states that the management
should take every defence to prevent a fire accident to occur and necessary awareness classes should be
provided to the workers. The management should consider every chance of fire and act suitably. The
management should also provide necessary equipment to prevent fire accidents and also provide adequate
training to the workers to use these equipment.

Consideration of likelihood AND severity

Likelihood of occurrence of fire accident is very high. The grinding works, welding and other uncovered
flame works are carried out daily in the workplace without those activities the whole site works cannot be
continue and it is taking place near the flammable liquid storage, since there is no other designated storage
place to store they are kept near the work area. The uncovered sparks may activate the fire and likelihood

of causing explosion and fire in the workplace is very high. The continuous working of equipments can
cause spark that can lead to fire if it caught to nearby combustible material. All workers plant operator,,
supervisors, site managers and visitors in the plant will be affected. .There will be high chance occurrence
of accident for 60 workers cause 60 worker are working with machinery operating and welding and
drilling work. Their work shift is 9 hrs and working hours are 8 hours. There is a high chance of accidents
occurring in the 8 hours.

Severity is of 4 categories, they are

● Minimal- no injury or no damage

● Minor-injury that need first aid attention with less considerable or negotiable damages to plants
● Major-injury that needs hospital treatment/heavy damages to plant and equipment.
● Catastrophic-accident causing death/irreplaceable damages to plant and equipment

The Severity of the risk can be categorised as Major to Catastrophic . When fire occurs at the plant it can
lead to severe injuries such as skin burns, suffocation and unconsciousness. Smoke from the fire will damage
the heart and lungs and in some cases can cause death.. If a large fire occurs, it will increase the number of
fatalities. Also it causes severe damages to the property and equipment and causes environmental damages.

How effective the action is likely to be in I suggested constructing separate storage areas for the flammable liquids . In Your organisation flammable

The action will not fully control the risk but storing flammable chemicals in a separate area will reduce the
chance of drilling and welding sparkle and machinery equipment sparks splashes into the flammable
liquids. The action will control the risk at its greatest extent.

Justification for action 2

Action Purchase and use mechanical aids such as forklifts, trolley wheels and skates for lifting large and heavy
loads. ( hazard category: manual handling)
Specific legal arguments
As per the ILO convention C167 and recommendation R175 the employer must take all defences to
ensure that all the workers are working in a health and safety manner and to ensure the workplace is safer.

And ILO code of practices section 17.10.1 shows, the manual handling lifting of weights which presents a
health and safety risk to workers should be avoided by reducing the weight, by the use of supporting
mechanical devices or by other methods.
Consideration of likelihood AND severity
The likelihood of occurrence iof manual handling hazard is very probable. Injuries when manually moving
heavy objects or materials from one place to another. In many cases, workers were determined to be carrying
more than 50kg at a time. Also the distance of manual carrying was long and the steps involved in manual
movement were wealthy. Finally it can result in injuries to the worker who is carrying, as well as material
damage or loss.Poor manual handling techniques workers are lifting the heavy pipes and machineries in the
workplace to replace the one equipment to another equipment that can cause health problems. Workers who
were working under the section of loading and unloading and general labourers will be affected. 33 workers
are appointed for the manual handling works. They are working for 8 hour a day at this time the chance of
occurrence of manual handling hazard is very probable.

As per the severity classification given in the justification one we can categorise the severity of the hazard
is rated as Major because lifting and shifting heavy weight from one place to another can result in fatigue,
back pain, injuries, shoulder dislocation, musculoskeletal disorder, and other conditions that may necessitate
hospitalisation for long-term treatment. The equipment also can be damaged because of lifting heavy
machinery such as pumps and compressors.

How effective the action is likely to be in Providing mechanical aids for lifting large and heavy loads, the hazards correlated with manual handling can
controlling the risk. This should include: be reduced by implementing the control measure. It helps the workers to move the heavy loads from one

indicated in your risk assessment;
and
● whether you think the action will fully
control the risk. The task is given to the Purchase manager within the time period of 2 months and will be forwarded to the
project manager, and in light of the consequences, to purchase these items owing to funding approval and
supplier selection.

I think as long as mechanical aids are used by the skilled workers and are serviced and examined on a
regular basis, it can avoid the risks.

Justification for action 3

Action
Purchase and use gas detector before entering to the confined space (hazard category: confined space)

Specific legal arguments

According to ILO 167- safety and health in construction convention, 1988 (no.167) article 13,the
employer must take all defences to ensure that all the workers are working in a health and safety manner
and to ensure the workplace is safer.

According to article 19, the workplace must be secure with adequate ventilation so as to maintain an
atmosphere fit for respiration and to limit any fumes, gases, vapours, dust or other impurities to levels
which are not dangerous or injurious to health are within limits laid down by national laws or regulations
Consideration of likelihood AND severity
Likelihood of the hazards related to confined space is very high. There is no need to get down to the
manhole for any works. Because the maintenance and cleaning is not done before. So the workers have to
enter the manhole and there is a chance of inhaling toxic gases such as nitrogen oxide, hydrogen sulphide and
carbon monoxide. While doing work in the confined space for the piping and pumping and maintenance
work workers need to enter the tank, manhole and Inhalation of toxic gases and fumes produced in the
confined space due to the absence of proper ventilation It will severely affect the workers and checking
supervisors,Workers who work inside, especially welders. Engineers, safety officers when inspection time..

Learner number: Learner name: Page 25 of 27

So the likelihood is very high in the case of confined space accidents. The confined space worker and
supervisor including totally 15 workers are appointed for the confined space work and they are working for 6
hours a day in manholes and tanks so there is a chance of affecting workers in the 6 hours.

Severity of the rsi can be categorised as Major to Catastrophic, in the situation of confined space
injuries. While working in a confined space the workers supposed to inhale the toxic gases like hydrogen
sulphide, nitrogen oxide and carbon monoxide. It can cause issues like nausea, eye inflammation and loss of
consciousness. If the amount of toxic gas is 500-700 PPM, it will cause death due to asphyxiation.

How effective the action is likely to be in Before entering the confined spaces they have to test with a portable gas detector whether toxic gases are
controlling the risk. This should include: present. It will help to reduce the risk to a great extent. The detection of toxic and flammable gases is fast
● the intended impact of the action; and reliable. We can assume the manhole is safe or not by identifying the oxygen level and identifying the
● justification for the timescale that you safety from toxic gases.
indicated in your risk assessment;
and I gave this task to the Purchase manager within the time scale of 1 month. The financial department will
● whether you think the action will fully provide the source to buy the gas detector and the purchasing officer will find the best dealer to buy the gas
control the risk. detector. I hope the purchase manager will complete work within the allocated time period.
I think the action will not fully control the risk because gas detectors can help to prevent worker injury or
exposure to toxic gases. Gas detector will help to identify the poisonous gas and avoid the risk. But other
possibilities of toxic gases leak after the entry of workers So the action will control the risk at its greatest
extent.

Part 4: Review, communicate and check

Suggested word counts for each section:
● Planned review date or period and reasoning for this: 50 - 100 words
● How the risk assessment findings will be communicated and who needs to know the information: 100 - 150 words
● Follow up on the risk assessment: 100 - 150 words.

Learner number: Learner name: Page 26 of 27

Planned review date/period with reasoning As per my company policy. I should review the risk assessment each 6 months. I did my risk assessment on
08 – 04 - 2022 Then so I am setting the date of review at 02 – 10 – 2022.Even though I will make sure that
risk assessment is needed before the assigned date because of some unexpected conditions like,
➢ If introducing altered or new working methods or any new work equipment.
➢ If there is any change in curren following legislation or international standards.
➢ If any occurrence of accident or incident or ill-health effect in the workplace.

How the risk assessment findings will be I will arrange a meeting with all the management officials, safety officers, supervisors and engineers to share
communicated AND who you need to tell my findings with them about high and medium hazardous equipments and situations in the work site.I will
also request that the risk assessment summary and actions to be taken be posted on the company’s website and
on the safety notice board for easy access by workers. Also give some prevention measures and methods to
reduce the repeated accidents occurring in the workplace.
How you will follow up on the risk To monitor the progress of my risk assessment, I will set a reminder system 1 week before the end of the
assessment to check that the actions have action. I will contact the person responsible for each control action in my risk assessment and ensure that the
been carried out
action is completed within a certain timeframe, if an action is delayed then I will ask the reason why the
action was not completed on time this can sometimes be caused by lack of skilled workers for certain
activities or lack of financial resources for management.I will find the reason, for example financial reasons
or other resource issues. If I find any failure in the safety procedures then I will take that into the attention of
the organisation’s authority to avoid unwanted hazards and advise the authority to take action against anyone
who is violating the safety procedures.
