PLATFORM SWITCHING CONCEPT

• The concept is always bone protect itself with periosteum and connective
tissue and C.T. covered itself- protect with Epithelium .

Tooth
 Junctional Epithelium
 Gingival fibers
 Periodontal Ligament
Implant
Junctional Epithelium
Gingival fibers
Ossiointegration
JE is below the A-I interface

Thebiological width forms as a defensive mechanism against the bacteria, influences the

remodeling of soft and hard tissue around implant andclinical aspects in dental implantation.
The formation of biological width around implant is a complex process
after severalweeks of healing. The biological width around implant is a
3-4mm distance from the top of the peri-implant mucosa to the first
bone-to-implant contact
• Classic Banamark Implant – without Platform switching .
• the result always 1.5-2 mm from the abutment-implant interface to the
first bone-to-implant contact
The influence of different variables :
 the surgical technique
 the surgical protocol,
 implant loading
 implant structure
 titanium surfaces
 abutment materials and Design
 Platform S.

on peri-implant biological width has been evaluated.

• If you place the implant below the bone

level >> the bone hate mictogabs
microleakage and micromovment and
goes down .
• In non-platform S
We need to get supracrestal biologic width
• One abutment one time if you keep take it on and off ou will have loose the
JE on abutment and loose Biological width . In non platform switch implant .
We have supracrestal adherence of soft tissue on the abutment .
The point its we have supracrestal fibers .
• See the biologicwidth on the abutment down to the shoulder and maybe
part of the way down if you take the abutment on and off .
• there has to be room to cover the bone from the outside .
• Hier is non platform S implants , We all know there is a horizontal component
to the biological width .
• The distance is 1.5mm = 3 mm wall between implants body . But bone resorb
• What is missing heir is the soft tisse not enough to keep bone the papilla is
short .
• The bone resorption (vertical distance ) is decrease down to half with
Platform S .
• The irritant is medialized by about 0.3-0.5mm
• Without Platform there is bone
resorption this 7mm implant
with 2mm bio wid formation .
• You loose 1/3 the length of
short implant
• If you put lateral –central implant without platform this will get short
v shaped papilla , 1.5-2 mm shorter then the adjacent papilla .

• In the pic the bone level is the same btw implant and implant (3
mm apart ) and tooth !!
• What we miss is the supracrestal biologic W . The tooth has B.W. on
the thooth supracrestal 4-5 mm next to the tooth .
• Papilla could be stabile in :
1- platform S .
2- no leakage .
3- one implant one time
4-supra crestal biologic width
