Instructions sheet: We encourage that you read this instruction sheet before using this tool.
OPT sheet: Use the OPT sheet to record the children's profile e.g. name, sex, weight, height, birthday and date of weighing.
•Make sure that you fill each line completely, including the name, sex, date of birth, date of weighing/measuring, height a
