RE: Request for Client File Retrieval - Oliver Muller

Dear Mr. Guerette,

I trust this letter finds you well. I am writing to request your assistance regarding
our mutual client, Oliver Muller. As of March 22, 2024, Oliver has chosen to retain
our firm, Ortega & Simpson LLP, for legal representation in his personal injury case
against Fake Coffee Shop Inc. Our firm's file number for Oliver's case is 9922A.

In order to proceed effectively with Oliver's case, we require access to his complete
client file from your firm. Specifically, we kindly request copies of the Plaintiff's
Claim, any medical records obtained, as well as all correspondence and other
relevant documents.

To facilitate the transfer of information, we have enclosed an Authorization and

Direction signed by Oliver, authorizing and directing your office to provide us with
the requested documents. We kindly request that you swiftly provide us with these
documents in order to assure Oliver's continued representation and to expedite the
