Using predicted posterior cornea (Barrett, EVO, Hoffer QST, Kane)

Currently outputting toric calculations for IOL cylinders of 1.00, 1.50, 2.25, 3.00, 3.75, 4.50, 5.25 and 6.00 diopters. Support for other IOL cylinder steps in
development.
Please double check toric steps in IOLs from B+L, SIFI, Zeiss, Ophtec, Simedice and Rayner since they are currently under testing.
