- Review JSAs, excercise Stop Work Responsibility and perform Hazard HSE officer

- Review if the tasks are related to the critical task lists.
- Any tasks diviated from the plan , SWA has to be conducted.
- PPE must be worn at all time particuarly the impact gloves.
- Prepare certified and functioning equipment prior execution.
- Confirm that there is no hydrocarbon / Mecury and ensure that gas
detection system is in place and functioning.
- If the hazardous gas / Mecury is shown, the safeguards such as the
crew's competency for hazmat traning needs to be in place.
- Ensure that the crane has been inspected prior to start of any shift.
- Check the weather condition prior to perform any task.

- Perform JSAs, V&V, crane checklist, lifting plan (including drop zone Task supervisor
diagram) and SWC for lifting & rigging and drop inspection prior to work.
- Review lifting plan, load chart and position the crane to have the highest
possible lifting capacity.
- Inspect working at height rescue equipment and ensure that the
equipment is ready to be used in case of emergency.
- Conduct SWC for working at height prior to work.
- Review the rescue plan and get documented on the location.
- Barricade working and lifting area.
- Inspect all handtools and lifting equipment before use.
- Use certifiled and correct lifting device with identification of SWL.
- Inspect and eliminate any loosen parts on lifting equipment/ loads.
- Beware of overhead loads, don’t stand under loads.
- Hands off load use adequate No's of taglines and push-pull sticks.
- Ensure that the scaffolds are certified and in date.
- Use a safety harness and tie with an appropriate securing point.
- Secure handtools with landyard.
- Use 2 person to lift and secure the clamps due to their weight.
- Do not stand on the rigless unit while skidding.
- A crane operator needs to monitor the weight capacity on crane load cell
display particularly when performing heavy lift.
- Perform short brief for the lifting plan prior laying down the jib crane from
vertical to horizontal.
- Ratchet must be used to tie up the CTJ with vessel during transportation
- Identify the area to put the jack in the vessel. (Vessel Layout)
- Keep walkway and area along lifting path clear. Task supervisor
- Prepare loading area on platform.
- Remove any objects causing trip hazar and highlight any trip hazards that
cannot be removed.
- No hands on loads.
- Use 4 taglines and push/pull tool to control the direction.
- Watch your step.
- Look for potential pinch points and hightlight them.
- Discuss and identify line of fire .
- Stay on proper position and ensure proper technique of body positioning
(e.g. applying torque).
- Ensure all doors and access ways are secured.
- Ensure that the proper torque has been applied with torque mark.
- Applying torque requires to have 2 person (e.g. torquing clamps).
-Weekly inspection on clamps MUST BE carried out to ensure the torque of
Hex Bolt and Hex Nut on each Clamp is still within the range.
- Gradually release sling tension before taken off.
- Slings must be aligned prior to lifting.
- Clean skid beams' surface and ensure that it's flat.
- Secure end bracing beams properly prior removing the slings.
- Perform Rig up connection inspection checklist.
- Lift the jib crane tower slowly.
- Check the vessel and crane position prior performing heavy lift.
- Pay extra attention when raising the jib crane tower.
- Turn on skidding alarm every time when skidding.
- Make an announcement prior skidding and after completed.
- Skid carefully and slowly.
- Ratchet must be used to tie up the CTJ with vessel during transportation

- Use radio with three ways communication. Task supervisor


- Check radios communication.
- Specify tasks in order.
- Confirm everybody understands the tasks.
- A banksman stays at the visible location for crane operator.
- A banksman gives orders in clear and correct signals to crane operator.
- Only banksman gives the signals except for any emergency cases.
- Clarify lifting direction to the crane oprator and captain.
- Prepare back up radios and batteries.
- Ensure a good commucation especially when the crane operaor cannot
clearly see the banksman's signal. If the communication lost, the crane.
operatior has to stop the lifting operation until the communication resumes.
- Spill kit needs to be onboard at all time. Task supervisor
- Clearly identify eyewatch station, first aid kit and spill kit's location.
- Verify crew's competency such as Haz Mat traning, H2S awareness
training.
- Absorbent matting needs to be placed under hose connection to absorb
any accidental spills.
- Ensure that the trip tank's draingage is closed properly.
- Spill containment kits to be used effectively to contain and clean up any
leaks and spills.
- Connections to be encapsulated with absorbent material and secured
with tie wraps.
- Inspect the lines for leaks on a regular basis.
- Supervisor to be informed immediately of any leakages/ spills.
- Barricade the area.
- A bucket needs to be in placed while bleeding off pressure or
disconnecting the hydraulic hoses.

- Bleed off the pressure trap before disconnecting the hose or fixing any Task supervisor
leaks.
- Stay away from line of fire.
- Verify that the safety pins, whipchecks and restraints are installed
properly.
- Verify all hose conditions is good and all connections is correct and tight
prior use.
- Maintain and record hydraulic hoses and air hoses condition by check list.

- Inspect all electrical equipment prior to use. Task supervisor
