Implementation

- It is not allowed to work on Mercury contamination during raining such as HSE officer
pull Mercury contaminated tubing or wrap tubing bundle.
- Clearly identify working zone (Hot zone, warm zone, cold zone).
- Canvas is required to spread to cover on main deck and cellar deck.
- A competent person will inspect the PPE for any damage before the team
members are allowed to continue with operations.
- Prepare the back up PPEs to be on location.
- Rearrange the upper deck for blowers and decon station location.
- Install decontamination shower.
- Make sure the site map is clear and the Mercury readings are clearing
marked.
- Perform the Pre & Post mercury measurement for all tasks, before and
after open well, thread protector, lubricator, BOP, pumping line, etc.
- The level of Mercury recorded will determine on the PPE required.
- Manage for crew rotation every 30-40 mins when operation with full face
and Tyvek suit.
- Monitor crew condition when wear the full face and Tyvek suit (Grey)
always, cooling vest must be used.
- Review JSAs, excercise Stop Work Responsibility and perform Hazard
hunt prior to start the job.
- Review if the tasks are related to the critical task lists.
- PPE must be worn at all time particuarly the impact gloves.
- Prepare certified and functioning equipment prior execution.
- Ensure that gas detection system is in place and functioning.
- Ensure that the crane has been inspected prior to start of any shift.
- Check the weather condition prior to perform any task.
- All crews work on mercury platform must perform pre-urine test with the
result less than 20 µg/g.
- Each shift is required to have at least 1 x safety officer.
- Personal landing area has to be at Cella deck.
- Barricade the area. Task supervisor
- Clearly identify eyewatch station, first aid kit and spill kit's location.
- Verify crew's competency such as Haz Mat traning, H2S awareness
training.
- Conduct Mercury survey and gas testing.
- Locate the blower to blow the mercury vapor out of the working area.
- If Mercury vapor level more than 0.025 µg/m3, required half mask and
Tyvek suit (Grey) during cleaning process.
- Cover grating and hatch cover with plastic to catch any fluids or Mercury.
- Lay plastic sheeting out around the work area or where any contaminated
equipment will be laid out.
- Preprare a drum of water at the work site to wash and dilute tools.
- Prepare Potassium Iodide (KI) and Iodine Crystals (I2) available on
location.
- Prepare 2 sets of SCBA on location for emergency rescue operations.
- If Hg contamination, wash affected area with fresh water, then soap,
water. Remove PPE and test effected person for levels of Mercury.
Effected person needs to take a shower.
- MSDS is onboard for all hazard chemical and decontamination solution.
- Place the secondary containment (tray) on the unit floors to prevent any
spill and droplet.
- Equipment and tools need to be washed in drum of water on site.
- It is not allowed to spot any contaminated equipment and contaminated
tools on the vessel.
- Removed TBG or CSG X-mass tree and W/H equipment, their open
ends need to be covered with plastic.
- Clean PPE and test with Mercury survey meter, if there is any reading of
Mercury put the PPE into a designated sealed storage drum in the
contaminated area.
- Spill containment kits to be used effectively to contain and clean up any
leaks and spills.
- Connections to be encapsulated with absorbent material and secured
with tie wraps.
- Supervisor to be informed immediately of any leakages/ spills.
- A bucket needs to be in placed while bleeding off pressure or
disconnecting the hydraulic hoses.
- Perform JSAs, V&V, crane checklist, lifting plan (including drop zone Task supervisor
diagram) and SWC for lifting & rigging and drop inspection prior to work.
- Review lifting plan, load chart and position the crane to have the highest
possible lifting capacity.
- Inspect working at height rescue equipment and ensure that the
equipment is ready to be used in case of emergency.
- Conduct SWC for working at height prior to work.
- Review the rescue plan and get documented on the location.
- Barricade working and lifting area.
- Inspect all handtools and lifting equipment before use.
- Use certifiled and correct lifting device with identification of SWL.
- Inspect and eliminate any loosen parts on lifting equipment/ loads.
- Beware of overhead loads, don’t stand under loads.
- Hands off load use adequate No's of taglines and push-pull sticks.
- Use a safety harness and tie with an appropriate securing point
- Secure handtools with landyard.
- Use ladder or scaffolding if the wellhead is high.
- Place the guard rail prior removing the hatch.
- Install and secure with the temporary removable guardrail or hard barrier
on the opening hatch.
- Do not remove the guardrail until the hatch has been closed.
- Keep tools or objects away from hatch.
- A crane operator needs to monitor the weight capacity on crane load cell
display particularly when performing heavy lift.

- Keep walkway and area along lifting path clear. Task supervisor
- Remove any objects causing trip hazar and highlight any trip hazards that
cannot be removed.
- No hand on loads.
- Use 4 taglines and push/pull tool to control the direction.
- Watch your step.
- Look for potential pinch points and hightlight them.
- Discuss and identify line of fire.
- Stay on proper position and ensure proper technique of body positioning.
- Gradually release sling tension before taken off.
- Slings must be aligned prior to lifting.
- Check the vessel and crane position prior performing heavy lift.
- Place the hatch cover properly to avoid any trip hazards.
- Only authorised personnel is allowed in the working area.

- Use radio with three ways communication. Task supervisor

- Check radios communication.
- Specify tasks in order.
- Confirm everybody understands the tasks.
- A banksman stays at the visible location for crane operator.
- A banksman gives orders in clear and correct signals to crane operator.
- Only banksman gives the signals except for any emergency cases.
- Clarify lifting direction to the crane oprator and captain.
- Prepare back up radios and batteries.
- Ensure a good commucation especially when the crane operaor cannot
clearly see the banksman's signal. If the communication lost, the crane
operatior has to stop the lifting operation until the communication resumes.
- Verify for any potential gas prior starting the hot work. Task supervisor
- Ensure that fire blankets are available and cover the hot work area.
- Ensure that the water supply is available included the fire extinguishers
are in placed.
- Assign a certified fire watcher.
- All gas detectors are certified and performed the daily bump gas test.
- Check hazardous gas level around working area prior start operation and
keep monitoring.
- Place the gas detector station close to the hot work area for monitoring.
- Check condition of oxygen/acetylene cylinders, gauges, hoses and that
flashback arrestors are installed at both cylinder and torch.
- Be aware of any sparks which might fall down underneath the working
area.
- Hot work permit is included.
- Use foaming agent to prevent fire.

- Verify that all PCEs is certified and valid. Task supervisor

- Barricade the area prior performing pressure test.
- Verify that there is no pressure between all annuli and hanger seals.
- Secure hydraulic source and ensure that production lines are isolated to
the well which we are working.
- Use correct ring gasket and riser spool adaptor.
- Stay away from line of fire.
- Verify that the safety pins, whipchecks and restraints are installed
properly.
