This is to inform you about the unfortunate passing of the insured,___________, on (Date of
death) under Policy Number__________. The insured’s untimely demise during the
contestability period lead to the non-utilization of the memorial plan.

Understanding the memorial plan is within the contestability period, I am aware that the
company still conducts a thorough review during that time. Due to that circumstances, our family
opt to avail the memorial service from other funeral home. Regrettably, the memorial plan
transfer could not be submitted earlier due to the unavailability of key family members needed to
complete the necessary paperwork.

conditions outlines in the contract and assure of my cooperation throughout this period.

While the policy has not been utilized as of now, I intend to keep it in force for its intended
