Julie Rose Escobar is authorized to obtain the birth certificate on the following conditions:

To present a valid identification card for verification purposes.

To sign any necessary documents required by the PSA.
To receive the birth certificate and any related documents on our behalf.
I understand that by issuing this authorization, I am legally permitting Julie Rose Escobar to act on our
behalf regarding the retrieval of our birth certificate from the PSA.

Attached to this letter is a photocopy of our valid identification card for verification purposes.
