PARENTS’ UNDERTAKING FORM (Gr. 3 to 12)

Code of Behavior of Students in General Education Institutions

As per the directives of SPEA, the student and the parent shall sign an undertaking at the beginning of
the scholastic year by agreeing to review the contents of the Code of Behavior Management for students.

The general policy of the Code of Behavior of Students states that all students, school staff and
guardian shall have roles and responsibilities that all shall comply with in order to ensure personal,
cognitive and social upbringing to the student in a safe and secure environment, through applying
this regulation properly.

Based on the above, the student and his / her guardian shall acknowledge that they have been informed
of the Code of Students Behavior and shall agree to respect its provision and comply with all provisions
contained therein. Copy of the same is uploaded on the school website (or Click here for details).

Kindly fill-in the Undertaking below and submit to the respective class teacher on/before 16th May 2024.
