GUIDELINES AND RUBRICS ON AWARDS AND RECOGNITION


SY 2016-2017

I. Grade-level Awards

A. Academic Excellence Awards


 This is given to learners regardless of section who have attained a
General Average of at least 90 AND a passing Final Grade in all learning
areas. (see page 4, B.1)
 The General Average is reported as a whole number (DepEd Order 8,
s.2015).
 The class advisers will give to the AC the list of qualified learners to be
awarded. (Date of submission: _________________)
 The class adviser should submit for evaluation the Form 137 and Report
Card of the learner (see page 12, below Table 9).

B. Leadership Award
 This is given to learners in Grades 6, 10, and 12. (see page 4, B.2)
 This is awarded during the completion or graduation ceremony.
Requirements: (see page 4, B.2)
1. Have no failing grades in any of the learning areas.
2. Have not committed any offense punishable by suspension or
higher sanction according to the Department’s service manual
and child protection policies in the current school year.
3. Be a class officer or an active member/ officer of any
recognized school club, team, or organization.

C. Award for Outstanding Performance in Specific Disciplines


 These awards are given to learners in Grades 6, 10, and 12. (see page
5, B.3)
 There may be more than one category of awards under the following
disciplines: Athletics (Track, Archery, Basketball, Futsal, etc.),
Communication Arts (English, Filipino, etc.), and Tech-Voc (Cookery,
CSS, etc.)
 This award will only be given to learners who have met at least 90%
of the criteria.

II. Special Recognition (see page 10, C)

 Learners who have represented and/ or won in competitions at the district, division,
regional, national, or international levels will be recognized.
 The schools may give due recognition to learners who have brought honor to the
school.
 The actual certificates, medals, trophies, and/or plaques received by the learners
shall be used to publicly affirm and acknowledge the contribution of the awardees in
giving honor to the school.
 This will be done during a flag ceremony or in a school-awarding ceremony.
III. What do awardees receive? (see page 10, III)

 The Grade-level awardees shall receive certificates, medals, and/or plaques from the
school, bearing the official seal of the Department of Education (DepEd Order No. 63,
s.2011)
 Schools are required to follow the specifications to ensure the quality of awards,
certificates, and medals and uphold the prestige of the awards.

 Certificates indicating the specific awards shall be given to all awardees. Attention to
proportion and detail (date and venue) is important.
 In case external sponsors, partners, and donors opt to give awards, they shall be
regulated by the school, subject to compliance with the policy guidelines. The awards
should be named after the trait, value, or achievement recognized, or an esteemed
person who is deceased.
 Schools must coordinate with external sponsors, donors, and partners regarding the
awards they wish to give the school. (see page 11, below the table)
