Pre-employment requirements

- Failure to submit any of the pre-employment requirements or falsification/tampering of the


same or providing false or fraudulent information may lead to revocation of job offer or contract
or if found much later, may be used as grounds for termination of employment.
- The Company keeps records concerning your employment, so it is important that your personnel
records (201 file) are accurate and up to date.

5. Tools of Trade

- Tools of Trade, including allowances, are intended for employees whose nature of work requires
them to travel regularly within and outside their home base (field personnel). These allowances
are not meant as compensation, benefit or perks and may be revoked or amended by the
Company from time to time.

An employee terminated for just cause, or who resigns without 30 days notice, shall forfeit the
unused vacation leave and sick leave credits, and shall only be entitled to the prorated unused
service incentive leave as defined under the Labor Code of the Philippines.

Sick leave and vacation leave credits that have been used to file for paid leave shall be deducted
from the final pay of the employee.

- Resignation
An employee who intends to resign MUST serve a WRITTEN NOTICE to the employee’s
immediate superior and to the Human Resources Department AT LEAST THIRTY (30) DAYS prior
to its effectivity.

questions about the policies, and I understand that any future questions that I may have about the
above or information provided in the New Employee Orientation and its contents will be answered by
the Human Resources Department. I agree to, and will comply with, the policies, procedures, and other
guidelines set forth in the orientation and any and all policies of the Company. I understand that
changes in the policies may supersede, modify or eliminate the policies as of the date I signed this form.
Changes to policies and new policies will be communicated to me by the HR Personnel through my HRIS
Account, or through official notices or, any other means of communication in the Company.
Notwithstanding the foregoing, I accept responsibility for keeping myself informed of these changes. As
an employee of the Company, I now commit to live by the rules, policies and the standards that the
Company upholds.
