At the end of this learning task, I should be able to:
1. Define school forms
2. Explain the importance and contents of the various school forms
3. Accomplish school forms required by the Cooperating Teacher
4. Submit required forms on time
5. Use professional reflection and learning to improve practice

Performing My Teaching-Learning Activities

Performance Task 1
Assisting the Cooperating Teacher in Accomplishing School Form 1 Class Register
Surf on the internet or ask your Cooperating Teacher to show you a template of SF 1 Class
Register. Study how it is accomplished by requesting your Cooperating Teacher to show you a
sample. Put at least five (5) entries into the template.

- Prepared by the class adviser
- The mode of preparation is LIS (Learners Information System)
- BoSY and as needed

School ID
A six (6) digit-number assigned to a school recognized in EBEIS
School Year
The prescribed period of time when schools offer daily instruction. It covers 10months of
regular schooling starting month of June (Beginning of School Year) up to the month of March
(End of School Year) of the following year.
School Name
Official name of school as registered in DepED and EBEIS
Grade Level A degree/stage of a learner classified according to age and progress. Section A
group of pupils/students convened together to receive instruction in a given course or subject.
Learner/ References/ Number (LRN)
Twelve (12) digit number which the pupil, student or learner shall keep while
completing the basic education program, regardless to transfer to another school or learning

Name of Learner

Name of an individual as reflected in the birth certificate or equivalent document seeking basic
literacy and functional life learning skills.

Sex (Male or Female)

Learner’s biological and physiological sex as reflected in the birth certificate or equivalent
document.

Birth Date

Date of birth of the learner as reflected in the birth certificate or equivalent


document.

Place of Birth (Province)

The name of province where the learner was born as reflected in the birth certificate or
equivalent document.

Age by June (1stFriday)

The actual age in years of a learner as of last birthday by the 1st Friday of each school calendar
year.

Mother Tongue

A group of people or homogeneous societies identified by self-ascription and


ascription by others, who have continuously lived as organized community on
communally bounded and defined territory.

IP (Specify ethnic group)

A group of people or homogenous societies identified by self-ascription and


ascription by others, who have continuously lived as organized community on
communally bounded and defined territory.

Religion

The religious belief of the learner, For example Muslim, “Islam” will be written instead of
Muslim under this column

Attendance
Surf on the internet or ask your Cooperating Teacher to show you a template of SF 2: Learners’
Daily Attendance. Study how it is accomplished by requesting your Cooperating Teacher to
show you a sample. Put at least five (5) entries into the template.

Performance Task 3
Assisting the Cooperating Teacher in Accomplishing Required School Forms
As a teaching intern, you might not be allowed to accomplish all the forms yet, however,
there might be some instances that you are requested to assist in preparing some school forms.
Make sure that you do it accurately and legibly (if done manually) and encode properly (if done
online). Review all entries/have it checked by your Cooperating Teacher before you finally
submit. It is best to have a reviewing copy (if done manually) and save the file (if done online).
Put a copy of the school form that you were requested to accomplish and have it signed
by your Cooperating Teacher.
It is best if you can get her comments/remarks and suggestions.
