The Philippine Stock Exchange, Inc. (“PSE”) is pleased to announce the launching of the PSE
Service Desk system, which can be accessed under the Online Systems of the PSE Website
(https://www.pse.com.ph).The PSE Service Desk is a portal where TPs can submit service
requests and report incidents through online forms. It automates the submission process and
facilitates the recording, handling, and tracking of requests and incidents. This automation also
aids in PSE goal of lowering its carbon footprint as part of PSE’s sustainability initiatives.

The PSE Service Desk implements a 2-step process where a TP personnel is required to create
the service request and another personnel is required to approve such request. In this regard,
PSE is requesting for TPs to submit the list of personnel who are authorized to submit the request
for services and list of personnel who are authorized to approve the requests. The authorized
approvers shall approve only the services that are available in the PSE Service Desk system and
will supersede the designated approvers in the Authorized Signatories Form (ASF) submitted for
year 2020. The ASF shall still be used and applied for services that are not included in the PSE
Service Desk system

The personnel indicated in the list will be registered in the PSE Service Desk and will be given
access to the system.

Effective Monday, November 2, 2020, all TP requests must be submitted thru the PSE Service
Desk, thus, printed and signed application forms shall no longer be accepted.

PSE SERVICE DESK REGISTRATION FORM


INSTRUCTIONS
1.0 Fill out the PSE Service Desk Registration form.
2.0 Upon completion, email the form to helpdesk@pse.com.ph.
3.0 PSE will collect, record, store, use, disclose, and process personal information consisting of
names, contact detail, and signature of authorized signatory, contact person, and employees
(“Data Subjects”) for the purpose checked/marked below and for purposes relevant or incidental
thereto. Said personal information will be retained as long as required by PSE and afterwards it
will be safely destroyed. The Data Subjects have rights under the Data Privacy Act such as rights
to be informed, access, correct, object, withdraw, erasure, data portability, file complaint, and
damages. They should be aware that there are risks in the processing of their personal
information and for safeguards to protect it, they can read the Privacy Policy at www.pse.com.ph
or contact the Data Protection Officer at dataprivacy@pse.com.ph for any concern.

Annex “C”
PSE Service Desk Registration Guidelines

1. The TP shall submit the PSE Service Desk Registration form, signed by the Nominee, to
register a TP personnel to the PSE Service Desk system.

2. A TP personnel assigned to create service requests or report incidents is called a “Maker”.

3. A TP personnel assigned to validate the requests submitted by the Maker and approve
the request for submission to PSE is called an “Approver”.

4. The designated Approver(s) in the registration form shall approve only the submission of
request for services that are available in the PSE Service Desk system and will supersede
the designated approvers in the Authorized Signatories Form (ASF) submitted for year
2020. The ASF shall still be used and applied for services that are not included in the PSE
Service Desk system.

5. Service requests require approval from the assigned TP approver and only service
requests that are approved in the system will be processed by PSE. The incident reports
submitted by the TP personnel do not need approval from the TP approver and shall be
directly sent to PSE.

6. PSE will assign a username for each of the Makers and Approvers indicated in the PSE
Service Desk Registration form submitted by the TP. The username and password will
be sent to the Maker and Approver’s email address as indicated in PSE Service Desk
Registration Form.

7. The TP may assign a maximum of five (5) Makers and five (5) Approvers. Any one of the
Makers may submit a service request and any one of the Approvers may approve the
service request for submission to PSE.
