CIRCULAR

Please find attached the Transfusion Reaction Reporting Form for Blood &
Blood Components and Plasma Products.
In compliance with the Heamovigilance programme of India as well as, as per
the direction of Drug Controller of India and NABH Guidelines,
1 Report Transfusion reaction to the Department of Transfusion Medicine
immediately after reaction is noticed by the doctor on duty, in the prescribed
format.
2. Provide two blood samples of patient one in plain vial and other in EDTA vial.
3 Also return the blood bag to the Blood Centre.

The above issued instructions are mandatory.
