Surgical site infection

• Infections of the tissues ,organs ,or


spaces that have been exposed by
surgeons during
Invasive surgical procedures.

• Infections occurong up to 30 days


after surgery or upto 1 year after
surgery in
patients recieving implants and
affecting either the incision or deep
tissue at
the operation site.
PIRO classification scheme

• Predisposition – Advancing age or immunosuppression


that may affect survival.

• Insult / Infection – It depends on the infecting organisms


or location of the disease.

• Response – It refers to the development of SIRS, shock,


elevated C- reactive protein, etc,.
According to depth :-

Types of SSI:
• Superficial SSIs – Affects skin and
subcutaneous tissue.
• Deep incisional SSIs – Affects fascia
and muscle layers.
• Intra-cavitary / Organ / Space SSIs –
Affects any part of the body other
than incision that is opened or
manipulated during any surgical
procedure.
Clinical features :

1.Superficial SSI
• wound is tender
• Erythematous
• Oedematous
Deep incisional SSI

• Tenderness extends
• Presence of crepitus
• Presence of vesicles
• Presence of bullae
Intra-cavity / organ /
space SSI

• This depends on the system involved.


• Example – Paralytic ileus continuing in
abdominal infections
