colon, proximal transverse colon with multiple mesenteric lymph nodes”, consisting of gray
brown to brown, rubbery, previously opened segment of intestine. The segment of intestine
measures 14.5 cm long and 8 cm in diameter. Cut section shows an irregularly shaped,
ulcerating mass measuring 8.0 x 5.0 x 1.5 cm, extending up to the muscularis mucosa and
involving the cecum. The mass is located 7.0 from the proximal line of resection (terminal
ileum) and 4.0 cm from the distal line of resection (transverse colon). The cecum measures
5.5 x 3.0 x 2.0 cm. The appendix measures 4.5 x 1.5 cm. The terminal ileum measures 5.0 x
7.0 cm. The ascending colon measures 5.0 x 8.0 cm. The transverse colon measures 4.0 x 8.0
cm. Cut section of the appendix shows slit-like empty lumen. Cut section of the cecum
shows unremarkable intestinal folds. Attached to the transverse colon is the omentum
measuring 8.0 x 6.5 x 1.0 cm. Attached to the ascending colon, up to the terminal ileum are
the mesenteric fats measuring 14.0 x 6.0 x 1.0 cm. There are ____ lymph nodes isolated. The
largest measuring 1.5 x 1.0 x 0.8 cm.

1. “Right and left fallopian tubes”, consisting of two segments of gray, rubbery, tubular
fallopian tubes, complete with fimbriae, each measuring 8.0 cm long and 1.0 cm in
diameter, and 6.0 cm long and 1.0 cm in diameter. The latter has a paratubal cyst located at
the midportion of the fallopian tube measuring 0.5 x 0.4 x 0.3 cm. Cut section shows patent,
unremarkable lumen.
2. “Placenta”, consisting of monochorionic, diamniotic placenta measuring 21.0 x 19.0 x 3.0
cm with two (2) eccentrically attached umbilical cords, Twin A umbilical cord measuring 26
cm long, and Twin B umbilical cord measuring 24.5 cm long. The fetal side is dull,
translucent, while the maternal side shows complete cotyledon. Serial section of the
placenta shows reddish brown meaty tissues with no areas of infarct nor hemorrhage. Cut
section of the umbilical cords shows three (3) blood vessels.

tissue measuring 28.0 x 25.5 x 12.5 cm with an attached tan ellipse of skin measuring 14.0 x
7.0 cm. The nipple is tan brown and protruding measuring 1.2 x 1.3 x 0.5 cm and is
surrounded by dark brown unremarkable areola measuring 4.0 x 2.5 cm in its widest
diameter. Likewise seen is a previous incision measuring 10.0 cm seen at the upper
quadrants, and is located 1.5 cm from the nipple. Serial section of the breast shows a white,
solid, mucoid surface, with areas of hemorrhages and necrosis.

 section shows an endometrial cavity measuring 2.5 cm long and 2.0 cm from cornu to cornu.
The endometrial lining measures 0.1 cm thick. The anterior myometrium measures 1.5 cm
thick while the posterior myometrium measures 1.3 cm thick. No mass noted. The cervix
measures 4.0 x 3.0 x 3.5 cm. Cut section shows the endocervical canal measuring 2.5 cm
long. Likewise noted are polypoid tissues along the endocervical canal.
