AUTORIZATION FOR RELEASE OF INFORMATION

I hereby authorize Idenfo Pakistan (Private) Limited and/or its agents sister concerns, and subsidiaries as defined
below to: conduct a verification of education, employment/work history, identity verifications, contact personal
references, search criminal history record information pertaining to me and/or receive an investigative consumer
report which can include character and/or general reputation. Every attempt will be made to keep the information
confidential.

This is further clarified that we will use the information you have given on your application form (together with any
information which we obtain with your consent from third parties) for assessing your suitability for employment. It may be
necessary to disclose your information to our agents and other subsidiaries. By returning this form to us you consent to
our processing personal data about you where this is necessary, for example information about your criminal offences.
You also consent to the transfer of your information to your current and future potential employers where this is
necessary (this may be to companies operating abroad if you apply for work outside of the Jurisdiction in question). By
signing below, you agree to this process and confirm that you do not have a criminal record subject to the current
Rehabilitation of Offenders Laws/Regulations and any amendments for the Jurisdiction in question.

I have read and understand this authorization for release of information and authorize the background investigation. I
authorize persons, schools, current and former employers, local name, ID number, current address, former address,
contact number, date of birth and other organizations and agencies to provide Idenfo Pakistan Private Limited and/or its
agents with any information that is requested, and I hereby release, and hold harmless, all the persons and agencies
providing such information from any and all claims and damages connected with the release of information. I agree that
any copy of this document is as valid as the original.

I HAVE CAREFULLY READ, FULLY UNDERSTAND, AND VOLUNTARILY SIGN THE ABOVE PERMISSION FOR RELEASE OF
INFORMATION.
