Dear Sir,

Anabella Maes is my patient, and has been under my care since 24 th December 2023. I am
familiar with her history and with the functional limitations imposed by her disability; she
has been diagnosed with Ovaritis which is inflammation of the ovaries that is responsible for
the production of eggs.

She has to undergo a surgical operation to correct the damage in her ovaries, this operation
has to be done very urgently because her womb might be damaged because of the
disintegration of the ovaries that is responsible for the production of eggs and she is to make
a payment of $800 for the surgical operation.
