would appear.

5. Provide certified or attested photocopies of all certificates, diplomas, degrees,

memberships, fellowships etc. and transcripts of examinations.

6. You may make your own BUET/REG.2 Form for preparing the application if you
feel so (use of the exact BUET/REG.2 Form format and structure is to be ensured)
