I, LOVELYN AGAPAY ASPREC, of legal age, widow, Filipino and a resident of Barangay 14
Nagrebcan, Badoc, Ilocos Norte, after having been duly sworn to in accordance with law,
hereby depose and say:

That I am the biological mother of KIDMAN ASPREC, LITTLEMAN ASPREC and

ADAN IRONMAN ASPREC who was born on March 5, 2006, June 10, 2008 and July 27,
2015 and they were born at Isabela.

That their biological father, MANNY B. ASPREC, my then husband, was murdered
why I became solo parent, since then up to the present I raised my child solely;

That I did not cohabit to a co-parent nor with another partner;

That I execute the foregoing to attest the veracity for all legal intents and purposes it
may be worth.
